Subject: Notice of Termination of Contract upon Expiration of Term

Dear Mr. & Mrs. ___________________o:

Our contract of lease over the premises located at


____________________________________________ is due to expire on June 15, 2021. Hence,
through this notice, the undersigned informs you of his intention not to further lease the
property beyond its term or after June 15, 2021 as he will devote the same for his own
business.

With that, you are hereby notified that you have until June 15, 2021 to occupy the mentioned
premises and advised to make the necessary preparation for transfer of place before the date
of turnover. The undersigned hopes mutual and good faith compliance of the conditions of our
lease contract throughout its term.

Please make arrangements for a mutually convenient time and date for a final inspection of the
condition of said premises, return of the keys and reimbursements if any upon turnover.
