ERECTION (Steel Structures)

JSA Title ERECTION (Steel Structures)


Minimum PPE’s are required (safety helmet, safety shoes, safety glass
Personal Protective and long sleeves) Erector and Scaffolders must wear approved and
JOB SAFETY ANALYSIS Equipment Required inspected Full Body Harness equip with double lanyard and shock
absorber.

BASIC JOB STEPS POTENTIAL HAZARDS RECOMMENDATIONS


TRANSPORTING MATERIALS

1. Loading of Fabricated Steel structure from 1.1 Material Falls 1.1.1. Use Approved crane with daily inspection
Fabrication yard 1.1.2. Use Approved Trailer truck
1.1.3. Ensure material fasten with inspected “lashing
belt”

2. Receiving material at site 2.1 Traffic Hazard 2.1.1. Good communication between the transporter and
the site receiver
2.1.2. Turn on Flashing light / Hazard light
2.1.3. Banks man posted and directing traffic
2.1.4. Ensure that the crane is set up on firm ground

3. Unloading Material using Crane 3.1 Material Falls 3.1.1. STARRT Form to be completed prior to
commencing the activities
3.1.2. Use Approved crane with daily inspection
3.1.3. Use softener (protection pad) to protect the slings
3.1.4. Use approved slings with quarterly colour code
3.1.5. Trained riggers to be used while unloading, tag lines
to be used to control the movement of the load
3.1.6. Crane operation to be barricaded, restricted access
to unauthorized personnel. Crane to be set up on
firm ground.

General Instruction:
- No defective crane should be in use
- No standing under a suspended load

PRIOR TO ERECTION

1. Slinging preparation 1.1 Sling damage and material fall may cause to 1.1.1. Use softener (protection pad) to protect the sling
catastrophic accident from damage
1.1.2. Use slings and shackles of the correct SWL for
unloading. Lifting gear to have the current quarterly
colour code displayed
1.1.3. Good communications between the rigger and the
crane operator rigger and the foreman in charge
1.1.4. Crane operation to be barricaded, restricted access
to unauthorized personnel
STEP PREPARATION

1. Preparation of workers, working at heights 1.1 Hazard to fall 1.1.1. Workers to be trained in the use of the Full Body
Safety Harness prior to commencing work
1.1.2. Provide access (green tag scaffold) for Accessing
the elevated area
1.1.3. Provide double lanyard (colour coded) for all
erectors and scaffolders.
1.1.4. Provide life lines (green tag/approval inspection)
1.1.5. Restriction (close barricades) of the working area
1.1.6. Minimum PPE’s should be worn all time
1.1.7. All materials and tools that will be use at heights
should be properly secure and tied with a rope

2. Preparation and handling of materials 2.1 Tripping Hazard 2.1.1 Good housekeeping and storage of materials
2.2 Injury cause by sharp edges 2.2.1 Working / leather gloves should be use

2.3 Human injury cause by improper lifting 2.3.1 manual handling training to be conducted prior to
the work commencing

3. Use of Mobile cranes 3.1 Defective equipment 3.1.1 Crane daily inspection is required

3.2.1 Crane daily inspection (check list) is required


3.2 Environmental and health hazard due to spill
General Instruction:
of diesel, oil, fluids.
Defective Mobile cranes should not be in use

CLIMBING AT HEIGHTS (INSTALLATION)

1. Using ladder when fixing the bolts and nuts 1.1 Slip, Trip and Fall 1.1.1. Use (3) three points when climbing ladders
1.1.2. Ladder must be in 75 degrees incline
1.1.3. Ladder must be 100 percent tied-off
1.1.4. Extra worker should hold the ladder to make sure
the stability.
1.1.5. All materials required at heights should be lifted
correctly (using ropes or crane lifts)
1.1.6. 1.1.6 Unused scaffolds shall be dismantled as soon
as they are no longer required
1.1.7. 1.1.7 Use Approved and inspected ladders,
displaying the current quarterly colour code

INSTALLATION OF LIFE LINES

1. Erection of Scaffolding as an access 1.1 Injuries due to Unsafe scaffold 1.1.1. STARRT Form to be completed prior to commencing
the activities
1.1.2. Only qualified scaffolders are to build, modify or
dismantle scaffolds
1.1.3. Scaffold must be inspected and checked by
competent person prior to use (tagging system)
General Instruction:
No one is allowed to alter the scaffold, only qualified
Scaffolders are allowed.

2. Installation of life lines 2.1 Hazard to Fall 2.1.1. STARRT Form to be completed prior to
commencing the activities
2.1.2. 100 percent tie-off to be maintained at all times
2.1.3. Life line should be made of cable wire and
clip/secure with (3) three “U” bolt at every end.
2.1.4. Life line should be Inspected and checked
(tagging system) by competent person prior to use

2.2 Material to fall


2.2.1. 2.2.1 Perimeter to be barricaded and a warning sign
positioned. Warning sign “Danger Overhead Work in
Progress”
2.2.2. All materials and tools that will be use at heights
should be properly secure and tied off with a rope
USE OF GAS CUTTING EQUIPMENT

1. Assembling Oxy-Acetylene Cylinder on Cutting- 1.1 Fire and explosion hazard 1.1.1. Ensure cutting-outfit is free of dust, no grease and
outfit oil not be used on the regulators or gauges
1.1.2. Flash Back Assertors are correctly installed
1.1.3. Only qualified persons to operate the equipment

1.1.4. Ensure that the correct PPE is worn for all activities
involving Oxy/Ace equipment
1.1.5. No combustible material near the work place

USE OF WELDING MACHINE

1. Welding works 1.1 Fire and Burns 1.1.1. Inspected Welding machine including all electrical
cables by competent person
1.1.2. Competent Welder is required to the task
1.1.3. Use all the necessary PPE’s for welding
1.1.4. No combustible material near the work place
1.1.5. Provide container can for all welding rod butts
1.2 Environment Hazard

2. Finishing work 2.1 Falling material and hazard to fall 2.1.1. All unused material; to be brought down from the
elevated area shall be lifted by either rope or crane.
2.1.2. Good housekeeping
