Minutes-Checking of Forms

The Sogod National High School Checking Committee conducted its checking of student forms on June 26, 2023. The committee validated the completeness and consistency of Forms 1, 2, SF9, SF10, and birth certificates for several classes and advisers. Some students were noted as having transferred schools or were no longer participating. Additionally, one transferee student to the 12th grade TVL-Home Economics program was found to have complete forms. No other issues or discrepancies were found across forms for the classes reviewed. The minutes were prepared and checked by members of the Checking Committee and School Principal.

CHECKING OF FORMS MINUTES


The checking of forms was conducted dated June 26, 2023 at 8:00 in the morning at the SBM Office
with the School Checking Committee. The consistency and completeness of Form 1, Form 2, SF9, SF10
with birth certificate and SF5A and SF5B has been validated by the chairman and members of the SCC.
The following notes and observations were taken:

ADVISER: ROSEMARIE B. BUBULI


GRADE 12-GAS (RIZAL)
- BONAAGUA, GLAICA B. was enrolled during the first semester under GAS, and was transferred
out to Vinisitahan NHS, ALS as of February 13, 2023. Records of the student such as SF10, SF9
was released to Vinisitahan NHS.
- BURCER, JOLINA B. was enrolled during first semester under GAS and did not enroll during
second semester SY 2022-2023.
- BARAQUIEL, JOAN was enrolled during first semester under GAS and has been marked as No
longer Participating in School as of October 2023.
- OSEA, PRINCESS YSABELLA M. was enrolled as transferee during 1 st semester under Grade 12
GAS. All forms of the student including SF10 from her pervious schools as well as SF9 (report
card) were validated as “COMPLETE”, and all subjects taken were checked and consistent to the
subjects offered by this school.

ADVISER: MAY M. BARCELONA


GRADE 10-INTEGRITY
- THREE (3) students were reported as TRANSFERRED IN namely: BIERSO, ALVIN, C. GAYSIS, EARL
JOHN B. and ENRIQUEZ, AMANDA. All have submitted their complete requirements such as
SF10 with birth certificates and SF9 from their originating schools.
- No inconsistencies/discrepancies of the School Forms checked.
- All students were marked as “PROMOTED”.
ADVISER: MARICEL B. BARUGO
GRADE 10-INTEGRITY
- No inconsistencies/discrepancies of the School Forms checked.
- All students were marked as “PROMOTED”.
ADVISER: MA. CHRISTINA B. EMBILE
GRADE 12-LUNA/MABINI
- 1 Student from 12- Mabini, named SINOGBA, VINCENT M., enrolled under TVL-Home Economics
was reported as transferee. All forms of the student including SF10 from his previous school as
well as SF9 (report card) were validated as “COMPLETE”, and all subjects taken were checked and
consistent to the subjects offered by this school.
- No inconsistencies/discrepancies of the School Forms checked.
- All students were marked as “PROMOTED”.
