AFFIDAVIT OF UNDERTAKING

I, JOSEPHINE MAILOM-RAMOS, married, Filipino and with office address at


Calauan Municipal Police Station, Barangay Silangan, Calauan, Laguna, after having been
sworn to in accordance with law, do hereby depose and say that:

I am bona fide member of Philippine National Police with the position of


Administrative Assistant III, designated as Crime Registrar and presently assigned at
Calauan Municipal Police Station, Laguna Police Provincial Office.

I am applying for Limited Private Practice of Law before the Office of the Chief, PNP
pursuant to PNP MC 2013-LS-003;

I hereby bind myself and undertake to comply with the following conditions set forth
by the aforementioned circular;

1. Represent Criminal cases only as private prosecutor;


2. Will only appear in civil cases or special proceeding not adversarial to the PNP as
a party;
3. Avoid conflict of interest;
4. Exert efforts not to prejudice public service;
5. Will not utilize that authority that would result in violation of the law on partisan
political activity;
6. Upon advice, cease in pursuing private practice if there is any indication showing
a tendency to violate PNP CMC 2013-LS-003;
7. Engage in limited practice of law for not more that two (2) days in a week; and
8. Be on-call on Saturday and Sunday to extend legal services to PNP units and
personnel needing legal services.

I attest to the truthfulness of this undertaking and submit to the legal and
administrative consequences thereof, if ever, the statements above are wanting in truth and
substance.
