Compliance Form –Workers Association

This refers to your application for registration submitted thru BLR Online Union Registration
(BLR-OURS).

Please be informed that we cannot continue with the registration process unless the following
documents with cross (x) marks are submitted:

For Workers Association:


1. Names of the applicant association’s officers and their respective addresses
2. Minutes of the organizational meeting/s
3. List of members who participated in the organizational meeting/s
4. Financial reports of the applicant association if it has been in existence for one
year or more years, unless it has not collected any amount from the members,
in which case a statement to this effect shall be included in the application.
5. Constitution and By-laws (CBL)
(specify lacking provisions)

6. Minutes of adoption or ratification of the constitution and by-laws and dates/s


of ratification (Minutes of adoption or ratification is not required if it is done
simultaneously with the organizational meeting and the same is reflected in
the minutes of the organizational meeting)
7. Names of ratifying members
8. List of documents that are not certified under oath by the Secretary or
Treasurer of the association, as the case maybe, and attested to by its
President, for the following documents:

To submit the lacking documents:

1. Go to https://blr-ours.dole.gov.ph

2. Write the username and password indicated in the email in the


Username and Password fields, respectively.

3. For first time user, write the old password, a new password, confirm
such new password and click Change Password. Always remember the
new password since it will be used whenever you login in the system.

In case you forgot the password, write the username and email then click
Forgot password?

An email shall be received indicating the new password. Go to https://blr-


ours.dole.gov.ph and use the new password to login.

4. Upon accessing the application form, go to the lower right message section
and click View to read the attached file sent by the DOLE Processor,
indicating the lacking document/s that needs to be submitted further.

5. Submit the required lacking documents indicated by the DOLE Processor by:
 Write a reply/message in the Comment Box to activate the attachment and
saving command.

 Click “Choose File”, select from your files the specific scanned document
required and click Open.

 Click Post to attach the selected file.

Repeat the process if there are several files to be attached.


Check if the message and attachment/s sent was posted successfully in the
Message Box. If not, repeat the process.

 Click Log Out to exit the system.

Upon submission of the above documents, the registration process shall continue and you will
receive an email indicating the date and the next steps to take to get your certificate of
registration. Please be advised that you have 30 (thirty) calendar days from receipt of this
notice to complete the above requirements, otherwise your application for registration shall be
deemed denied without prejudice to its re-filing.
