Fdar Psychiatric Duty

The document summarizes a nursing chart for a patient with schizophrenia. It documents the patient's disturbed thought process related to a lack of support system and inappropriate non-reality based thinking. It notes the patient said she had two children who were 80 and 90 years old, which is impossible. The nurse established trust, listened without agreeing or disagreeing, and tried to explain reality and bring the patient back to reality.
