PLEASE MAKE SURE WE ARE INFORMED IMMEDIATELY OF FULL DETAILS OF
OCCURRANCE WITH EXACT PERIOD AND BUNKERS COMSUMPTION (OR BUNKERS
ROB OF INCIDENT SARTED/ENDED) DURING THE INCIDIENT STATED AS ABOVE
IF HAPPENED. THE REPORT SHOULD BE FOLLOWED BY WRITTEN AND MAILED
TO THIS OFFICE SOONEST POSSIBLE.

LOADING/DISCHARGING STIPULATING THE EXACT FACTS AND IF THERE IS

TO BE INSERTED.
IN THE EVENT OF BREAKDOWN OF SHORE EQUIPMENT FOR CARGO OPERATIONS
SUCH PERIOD OF BREAKDOWN MUST BE CLEARLY MENTIONED AS BREAKDOWN
OF SHORE FACILITIES AND IF POSSIBLE PLEASE ASK THE AGENTS OR BY
YOUR SELF TO ADD THE NATURE OF SUCH BREKDOWN.
LOG ABSTRACTS
CHTRS WILL DELIVER YOUR GOOD SHIP THROUGH AGENTS AT A CONVENIENT
PORT SUFFICIENT BLANK FORMS OF THEIR DECK ABSTRACT AND ABSTRACT OF
ENGINEERS' LOG, AFTER COMPLETION OF CARGO DISCHARGING, PLEASE SUBMIT
FULL AND COMPLETED DECK/ENGINE ABSTRACT LOGS TO US VIA DISCHARGING
PORT AGENTS OR AT NEXT CONVENIENT PORT.

STEVEDORE DAMAGES
ANY STEVEDORE DAMAGES TO BE REPORT IMMEDIATELY IN WRITING TO THE
RESPONSIBLE PARTY WITH COPY TO US STATING DATE/TIME OF INCIDENT
AND/OR TIME WHEN DISCOVERED. PLEASE ALSO ADVISE US IMMEDIATELY
VIA TELEX IN ORDER TO EVALUATE IF PNI SURVEYORS ARE NECESSARY.
