Annex 1

Oath of Data Privacy

I, DINA P. VILLACERAN, a CONTRACT OF SERVICE WORKER of the Philippine


Statistics Authority (PSA), and assigned as the Screener hereby consents, accepts,
and commits to the following obligations, terms and conditions as set forth in this
Commitment:

1. I understand that in the course of my employment or engagement by PSA, I may


have access to all types of personal information about an identified or identifiable
individual, including sensitive personal information and privileged information,
collectively referred to as “personal data.” These personal data include those
contained in survey and registration forms, photographs, questionnaires, and any
other documents containing information about data subjects, including those in
digital form or stored as electronic documents.

In this relation, I agree that all personal data of registrants, applicants, and other
data subjects served by PSA, obtained or disclosed to me in the course of
performing my duties or those that I may have access to, whether intentionally or
accidentally, in the course of my employment or engagement with PSA, or by virtue
of my connection with PSA, are protected information.

2. I am obliged by law to adhere to data privacy principles, and to maintain the


confidentiality, integrity and availability of personal data that I gain access to, or that
I process as part of my duties. I understand and agree that by working in PSA, I am
obligated to comply with the following:

a. I will strictly comply with all policies, procedures, orders and guidelines issued by
PSA intended for privacy and data security;
b. I will be responsible for personal data that I process and I will exert reasonable
efforts to protect them against accidental or unlawful loss or destruction,
unauthorized access, modification or negligent handling and any other misuse;
c. I will not disclose protected information, share them in social media or similar
platforms, or take photographs or keep copies of records that come into my
possession, unless authorized by PSA;
d. I will not make changes or modifications to documents and records, nor bring
documents or any storage media containing protected information outside PSA
without proper authorization;
e. I will not use protected information for my own benefit or to the unwarranted
detriment of PSA or data subjects, such as providing access, giving, selling,
copying, disposing or permitting use by others.
f. I will not share my credentials such as my user account, username and
passwords, and I will be careful when sending or transmitting protected
information by electronic mail or online;
g. I will only access information that I need in the performance of my assigned
tasks and duties or that I have been granted authority to process;
h. I will comply with the provisions of the Data Privacy Act, and relevant laws and
regulations.

3. I am obligated to report to the Data Protection Officer (DPO) any unauthorized use
or disclosure, loss, theft, or misuse of protected information or devices where they
are stored, and any other incident that could lead to a privacy violation or security
breach immediately, upon knowledge or reasonable belief of their occurrence,
whether or not I am personally involved.

4. I will turn over all devices, storage media, and files, whether paper-based or
electronic, which I have access to or which are in my possession, to the designated
Annex 1

PSA officers, upon termination of my contract or separation from service, to the end
that I will not have in my possession or control any of these protected information,
or any documentation or other records from PSA.

I acknowledge that I will be accountable for my access and use of protected information,
including when I process the information unlawfully or without authority. I have the
obligation to maintain the confidentiality of all protected information that comes into my
knowledge even if I am no longer connected with PSA. I will be solely responsible for
any claims from third parties if such claim results from my own fault or negligence, or
failure to abide by the privacy policies and procedures in PSA. If any provision of this
Commitment is held to be invalid or unenforceable, all other provisions shall remain in
full force and effect.I also know and agree that my failure to fulfill any of the conditions
set forth in this Commitment and/or my violation of any terms of this Commitment shall
result in my being subject to appropriate disciplinary and/or legal actions including
termination of employment and claim for damages, without prejudice to possible
criminal liabilities under the Data Privacy Act.
