This document is a daily time record form for an employee to record their arrival and departure times from work each day for the month of October 2023. It includes spaces for the employee name, the month and year, arrival and departure times for regular work days and Saturdays if required. There are also columns to record any undertime (early departures) in minutes. The employee must sign to certify the times are true and correct and it must be verified by the employee's supervisor.
