A baby girl was born via emergency C-section to 29-year old Anusha O at 10:08PM on 03-10-2023 weighing 3.62kg. The baby cried immediately, had good APGAR scores, and was feeding well with stable vitals and passing urine normally. Examinations showed the baby was healthy with no issues. After 3 days in the hospital, the baby's bilirubin levels were below the phototherapy range so she was discharged in good condition.

Girl baby was borm on 02/10/2023 at 10:08PM by Emergency LSCS with birth weight 3.62kg. Baby cried
immediately after birth with APGAR of 9/10at I min and 10/10 at 5 min. Sugars were monitored and remained
normal with breast milk and Yormuia feeds. Baby is feedigwell, vitals stable, passing urine well. SBR done
after 36 hours was below phototherapy range and hence planned for discharge.

ON EXAMINATION
Baby is Acrocyanosis+,CRT- 2 sec, peripheries warm.
AF- flat. Eyes, Ears, Skull, Oral cavity- Normal.
Nocongenital anomalies/dysmorphic features
senitalia-Normal.
pine- Normal, Hips- Stable.
CVS: S1, S2+
RS: B/LNVBS+
PA: Soft
CNS: New born reflexes optimal

EXAMNATION AT DISCHARGE:
Baby is pink, CRT 2 sec, peripheries warm.
AF- lat. Eyes, Ears, Skull, Oral cavity- Normal. Red reflex positive.
Genitalia- Normal
Spine- Normal, Hips- Stable.
CVS: S1, S2+
RS: B/L NVBS+
PA:Soft
CNS: New born reflexes optimal
