1 PURPOSE

The purpose of the document numbering procedure is to provide a standard methodology and
guidelines for corporate/project specific of following entity:

i. SUBCONTRACTOR
ii. VENDOR

2 DEFINITION

CHARTERER Represent MEDCO E&P Natuna Ltd. (MEPN)

OWNER Represent Consortium Hanochem – Cakra - MTC.
CORPORATE DOCUMENT Defines as documents/forms produced for management /
administration works related to COMPANY’s operation and is
independent from WORK CODE specified in Appendix 1.
ENGINEERING DOCUMENT Defines as deliverables produced by Engineering division.
MANAGEMENT DOCUMENT Defines as deliverables produced by entity other than
engineering discipline.
VENDOR Defines as original equipment manufacturer / consultant /
contractor appointed by OWNER.
VENDOR DOCUMENT Defines as document produce by VENDOR as specified in
Appendix 5.

3 CORPORATE DOCUMENT NUMBERING SYSTEM

All standard documents forms produced are identified by a unique numbering system. The
Document Controller and Document’s Originator will confirm the allocated identification number
of the forms and attachments

Standard identification numbering system for forms and attachments, with extension to
procedures number, as the following :

Otherwise, numbering system also can be used for Project, Tender, Proposal, with extension

5 VENDOR DOCUMENT NUMBERING SYSTEM

COMPANY document controller shall assign a unique number to each vendor and sub-vendor
documents. VENDOR document numbering shall adhere to numbering sequence as below.

Deliverables submitted without correctly titled / numbered and badly formatted scanned electronic
files will be deemed unacceptable by the COMPANY document controller and returned un-
reviewed for amendment.

REVISION NUMBER Revision number as per appendix 1.Revision number does
not form part of the unique document number.

6 REVISION CONTROL

Document revisions shall be alphabetic, or numeral assembled from the life cycle state Code
and incremented each time the document is issued for a given life cycle state. The revision code
is different for each phase.

On occasion documents will be issued to third parties for information purposes only which
require no further action by the recipient. These documents will not have a specific revision
code upon issue, but their purpose will be indicated on the transmittal from Company by the
Status Identifier “Issued for Information

7 ELECTRONIC SUBMISSION FORMAT

i. Document shall submitted submission as electronic files in the following preference :

- Adobe Acrobat (.pdf)
- Native file Format (.dwg,.xls,.doc, .etc)
ii. The electronic file name shall contain document number and its revision number in the
following format : <document number>_< revision number> as shown below:
iii. Adobe PDF files should be submitted in the format of rendition, where possible, rather
than a scanned PDF image. PDF files should be rendered directly from the authoring
application to allow for easy view, search and later retrieval.
