The Area of the land allotted for the project is around 36.35 acres. Proposed Built-Up area of the buildings is around 98,000 Sqm. Project cost is INR. 354.39 Cr. Audit Obj For the safety audit team, all responsible officials of contractor, as declared during the weekly review meeting dated 8" Dec 23, were invited for this exercise but only one officer-Mr. Farhat Abbas ( Safety officer, MCL-SIIPL) joined us. The audit aims to assess and evaluate the compliance to safety protocols at the site. Having identified the gapes, more organized strategy can be formed by the contractor to address & monitor all issues pertaining to non-compliance consistently. In such away, the Safety, Heath& Environment performance at the site would get enhance. Various safety violations observed at site: 1. Hard Barricading not available- a) COSA A1, A2, B1, B2 water tanks and staircase slab area. b) Academic A-B, B-C and their connecting corridors ©) Steel bridge (All 04) need side temporary barricading d) Girls & boys hostel staircase & lift slab area, e) Trainers hostel all windows and door openings. Wy] 2. Electrical & Fire Hazard- a) Wire found damaged in most places at the site. b) Uses of wire tops are completely eliminated from the site. ©) ELCB/RCCB has completely vanished from the site. No short circuit protection is available. 4) The electrical distribution box is thrown here and there and is not kept ) in a safe location; also, all-weather protection is not available with them. €) At our site most of our locations using naked domestic wire without having plug top. This raises the likelihood of electrical and fire hazards. As per industrial safety norms only industrial, double insulated 3 pole wire and plug tops can be used. f) Where hot work is in progress, NRV, flash back arrestor, and fire extinguisher is not available, which increasing the risk of a fire incident on site. Page Lot3. Housekeeping- a) Plastic, wooden, steel, paper, and other scrap are thrown everywhere at the site. It is observed that plastic and other C&D waste are buried with backfiling, which is not safe for the environment. b) Due to poor housekeeping in side buildings, it is difficult to ensure the safe movement of workers; on a daily basis, tripping incidents are increasing, ¢) It has been observed that after cleaning buildings from the inside, dust and construction waste are thrown out the windows, increasing the risk of injury and contaminating the air. 4. Lifting machines & cranes- a) The third-party inspection ( TPI) certificate of the Mobile Tower Crane (03) and monkey hoist crane (09) and other lifting tools and tackles expired on Nov-Dec 2023. b) A damaged lifting bucket is being used at the site ( boys hoste! - MTC) ©) Hook safety latch is not available with any MTC or monkey hoist crane at site. d) A full-body hamess is not being used by the monkey hoist crane operator at the site. @) Cradle inspection is not done daily, weekly, or monthly. #). Crane operator competency certificate is not found at site 9) Crane operator health fitness certificate is not found at site. 5. Personal Protective Equipment’s (PPEs)- a) Work-related PPEs is not available on site. Hand gloves, nose mask, ear plug, shoulder pad, and safety goggles are not available on site for more than 6 months. b) Only safety shoes, a safety vest, and a safety helmet are avaitable, which is also not as per Indian industrial safety standards. 6. Site Sanitation- a) At the site, only four toiles are available in working condition, which is not sufficient for the present manpower at the site. b) Due to a shortage of urinals and toilets on site, workers are urinating in open spaces and most of the time inside the buildings, which will develop a fusty atmosphere. It is also a hygiene issue. 7. Scaffoldi a) The scaffolding erection standard is not followed at site, Most of the time, scaffolding is tied with plastic rope, which is not rigid enough to tackle the load of scaffolding and is unsafe for work. AS per safety standards, all scaffolding should be tied with a metal clamp only. b) Scaffolding inspections are not done on site. The first inspection should be done immediately after the installation of scaffolding, and regular inspections should not be done as desired on a weekly basis or as per requirements Page 2 of 48. Work at height- a) A single lanyard full body hamess is used on site, which is not allowed. Only @ double-lanyard full-body harness is allowed. It is also observed that the ropes of such belts are ruined and tied with binding wire, which is not safe for ‘work at site b) The painting and texture team is not using a walkway platform to work on site, which is unsafe for work The plastering team is not binding the platform with the scaffolding ©) Safety net installation is pending on COSA At-A2, B1-B2 water thank & staircase slab area. Girls, boys, trainer's hostel staircase and lft area 9. Vehicles- a) Buckhorn is not available or working on any vehicles on site. Also, front homs are available and work with a few vehicles only. b) The motor cover (water pump and transmission motor) is not available in the transit mixer. ©) Oil and diesel leakage is observed in most vehicles at the site, which may lead to a fire hazard. Also, due to oil leakage, water and soil contamination and environmental pollution increase. ) Poor maintenance of all vehicles and machines is exhausting excessive amounts of smoke, which is increasing the air pollution at the site. ) It is observed that driving license, PUC, insurance, fitness certificates of the vehicles not available at site f) Driver! Operator medical fitness and competency certificate not available at site. 10. Labor colony — a) Workers have complained about the labor colony not being cleaned on a regular basis, and there aren't enough toilets, b) There is no arrangement to collect kitchen waste from the labor colony, a few dustbins are available, but they are insufficient in number; and dustbins are not regularly cleaned c) Water leakage and tank overflowing observed many locations. d) The electrical distribution box is mounted on the colony's wall, increasing the risk of electrocution.(Because the labor colony is made of metal sheet, workers may be seriously injured if they come into contact with any open electrical wire. e) LPG is not provided for cooking, the majority of workers cook with wood. f) Because there is no centralized mess/cooking facility at the labor colony, all workers cook inside their living rooms, increasing the risk of fire, electrocution, and suffocation. 9) Créche facilities are not available at labor colony. 11. Miscellaneous — a) Workers and staff identity card not issued by MCL-SIIPL. Page 3 of 4b) Workers medical fitness certificate is not available at site. ©) Medical facility is not sufficient, a first aid box is available but no trained first-aider is available to facilitate at the time of injury, d) It is also observed that ambulance/ emergency vehicle is not available at site e) Job safety analysis (JSA)/ HIRA is not preparing for high risk activities. Please note few JSA is available for example- plaster, brick work etc. f) The permit system is not sincerely followed at site, the safety officer and other site engineers simply sign the permits without assessing the site conditions, they siting at office & sign permits. But as per standard safety PTW procedure, all permits should be issued only after physically verification the site conditions and meeting all safety compliancas, but this is not the followed at the site. 9) As discussed with worker they complaint they were not paid the same wage for the same work at the site.
