REFLECTIVE JOURNAL
1 SEMESTER S.Y. 2023-2024
st
General Objective:
To put my knowledge and skills to work in providing care; it also helps me to improve my abilities and
have a better understanding of providing care in the Community Health Nursing setting.
Specific Objectives:
Individual professionalism is demonstrated by personal behaviours and appearance.
When giving care to individuals and families in the community, use analytical reasoning and
critical thinking abilities.
Use a range of ways to effectively communicate with varied groups and disciplines about the
health needs of individuals and families in the community environment.
Collaborate with clients, significant others, and to my community health nursing team
colleagues.
Recognised the impact of economic, political, social, and demographic forces on health-care
delivery.
I learned that a community health nurse is not the same as being a hospital nurse. In comparison
to hospital nurses, community nurses have greater medical autonomy. They represent medical
authority in the home, school, or even the community as a whole. Community nurses, as compared
with hospital nurses, where they can communicate with the physician for managing the patient’s care
and are completely accountable for a patient's care and may make independent decisions based on
the patient's condition, such as referral. As community nurses, their primary focus is on health
education, advocacy, community assessments, and disease pattern studies, as well as being the first
line of defence during disease outbreaks. They educate community members about managing chronic
conditions and making good choices, preserving their health so that diseases, disabilities, and
fatalities are reduced, and conducting research to improve health care.
Aside from that, they have the opportunity to develop long-term relationships with community
members and conduct community visits several times per week or month, during which time nurses
identify and recognise community problems, plan effective interventions to solve the problems, and
monitor any changes in behaviour. Indeed, being a part of this group taught me the true meaning of
community health nursing. It has greatly aided me in understanding common health problems or
issues found in the community, which may be of great assistance in honing my skills in providing care
for people in underprivileged areas. For example, we have dealt with families who have Hypertension,
Arthritis, High Blood Sugar, and had recently undergone surgery. Each group was given the
opportunity to present and discuss the many concerns. We also had forums where we could ask
questions about our Clinical Instructor, which I find very useful because it increases our learning
experience and allows us to acquire insights into the issue being presented.
One of the most rewarding aspects of being a student nurse is the unique role we play with those we
care for. Our job encourages clients and their families to share things to us that they might not be
willing to speak under normal circumstances. Furthermore, nursing is a career path that has
enormous duties as both a compassionate health care provider and a conscientious advocate. This
semester, I got to see nursing from the perspective of a Community Health Nurse in Barangay Capo. It
has given me the opportunity to put my nursing skills to use in a variety of ways.
