WOUND+ULCER

Is any gap /discontinuity of the skin by either physical trauma or mechanical trauma or spontaneously or
pathological is called WOUND/ULCER

CATEGORY OF WOUND

Abration

Laceration

Bruise/contusion

Crashed wound

Abration:is the removal of only epidermal layer of skin. Have tendency of forming pine point bleeding eg
abration at right distal caused by blant object

Bruises/contusion: is the swelling of body after trauma

There is bleeding interval

Cause of wound

Blanton object/ unpinned object

Laceration

Is the linear wound. Always caused by sharp object the wound may be deep but clear

Avution: is the developing by glowing of skin, and sometimes can be turn back to its normal condition

Crashed wound: it is deep wound and dark ,caused by blant object it is vice versa of Laceration some
part of skin are being loosed

Occur on slidding condition

Tied wound
Wound always caused by sharp object

It have low risk of inf

It can heal by Primary intention

Clean by looking

UNTIED WOUND

it caused by blunt object

Have high risk of contamination

It can heal by Primary or secondary intention but mostly it healed by secondary intention because it is
deep wound which takes long duration to heal so we do skin grafting

ULCER

All ulcer are wound but not only wound are ulcer

Characteristics of ulcer

margin- >border line of the wound

Floor->exposed surface of ulcer

Edge->connection between margin & floor or distance

Base-> site where floor lie on it

Type of edge

Slopping edge

U can't differentiate between margin and floor -have serous tissue

it Occur on healing wounds

There is no any dead tissue-> are red in colour which contains only granulation tissue

PUNCHED OUT EDGE: have very straight deep edge

Occur non healing wounds. Have dead tissue which whitish which pus production (died tissue on floor
=sluphs)

Raised edge/court flower: Occur on malignant ulcer


Rule out edge :syphilitic ulcer (ulcer caused by Syphilis

Criteria of Base to rule out malignancy

On normal when you shaking the wound must be Sharken but on malignancy ulcer are not shaken when
they are formed directly on bone

Clinical classification of ulcer by looking (through its features)

Healing ulcer

Have Slopping edge. Floor contains granulation which serous tissue margin can be regular or irregular

Spreading ulcers

Margin can be regular or irregular in shape it is non healing ulcer its edge must be undermined edge or
punched out edge

Floor contain serousgious discharge (combination of blood and pus

Have foul smell

Malignant ulcer have raised edge sometimes contains slaps or not

Easily bleeding on touch. It's Base is fused to the bone

Factor that facilitate /speed up wound healing

Edge

The more the young the more good healing because of formation of new cells

Site of ulcer :wound near heart have more healing

Size of wound smaller

Good Hygiene

Good diet

Protein for formation of new cells

Vitamins formation of new tissue

Factor that hinder wound healing


Poor Hygiene->poor diet->to be aged ->co-morbid (co-existing disease )->large size of wound
