ACKNOWLEDGMENT AND CONSENT

CONSENT FOR THE CREATION OF SCHOOL-OWNED EMAIL FOR MY WARD

PLEASE READ THIS CONSENT CAREFULLY AS IT CONSTITUTES A LEGAL


AGREEMENT BETWEEN SHIV NADAR SCHOOL AND THE STUDENT’S PARENT
AND/OR LEGAL GUARDIAN.

This Acknowledgment and Consent (this “Consent”) must be signed by the parent or guardian
of (1) a student under the age of majority; or (2) an adult student who does not have the
competence or capacity to consent. A copy of this signed Consent will be maintained by the
school and a copy must be transmitted by the parent or guardian.
In consideration for creation of e-mail ID for their ward, the undersigned parent or
guardian consents and agrees to the following terms and conditions:

SECTION A: STUDENT EMAIL AND COMMUNICATION POLICY


By signing this form, I consent to the creation of e-mail ID for my ward on behalf of whom I
am signing this form. I have read and agreed to the Student Email and Communication Policy
(“the Policy”) which can be accessed by clicking here.
If I have any questions about the Policy, including my or my child’s rights under that Notice, I
have been advised to contact Shiv Nadar School’s Data Protection Officer (Grievance Officer)
by email at dpo@sns.edu.in.

SECTION B: ACKNOWLEDGEMENT AND AGREEMENT CONCERNING THE


EMAIL ID
I acknowledge having read and understood the information contained in the Student Email and
Communication Policy as on Shiv Nadar School website, which can be accessed by clicking
here, and having received sufficient disclosure of the methodology of the Shiv Nadar School
in order to make an informed choice about creation of an email ID for my ward. I acknowledge
and agree that I have been advised about the rights, responsibilities, and duties of the student
with email ID in addition to the functions and usage of the email ID and the procedure for
redressal of my or my ward’s grievances.
I acknowledge that Shiv Nadar School will supervise the functioning of the email ID form of
usage and incidental matters for the safe and smooth functioning of the email ID. I acknowledge
that the email ID created for and provided to my ward is and shall remain the sole and exclusive
property of Shiv Nadar School.
By signing this form, I acknowledge and confirm, on my own behalf, and on behalf of the
student for whom I am signing this form, that I have read, understood, and accept the terms and
conditions with respect to my or such student’s access to, use of the email ID, including,
without limitation, the terms and conditions that are set out in the Student Email and
Communication Policy (“the Policy”) which can be accessed by clicking here.
In consideration of the consent for my child for email ID, I agree (1) to keep confidential and
not disclose, divulge, share, distribute, publish, or otherwise make available to any third parties
any information relating to the email ID (or the information contained therein) relating to Shiv
Nadar School that are provided to me or my child in any form or media in connection with the
Shiv Nadar School (collectively, the “SNS Materials”), and (2) to only use the SNS Materials
directly in connection with my or my child’s participation in creation of email ID. I further
acknowledge and agree that any unauthorized use, disclosure, distribution, reproduction,
copying, sale, lending, public performance, publication, dissemination, making available, or
broadcasting of any SNS Materials without written permission from Shiv Nadar School is
strictly prohibited.
