Email Parents Consent Form
This Acknowledgment and Consent (this “Consent”) must be signed by the parent or guardian
of (1) a student under the age of majority; or (2) an adult student who does not have the
competence or capacity to consent. A copy of this signed Consent will be maintained by the
school and a copy must be transmitted by the parent or guardian.
In consideration for creation of e-mail ID for their ward, the undersigned parent or
guardian consents and agrees to the following terms and conditions:
