1. ZETO RULE
A .What is your understanding of ZETO Rules?
ZETO means Zero Tolerance. To be follow and comply at all times. To ensure all activities are carried out
in a safe manner and non-compliance is not tolerated.
B. How many ZETO RULES?
10 rules. (7 DO’s Blue colour and 3 Don’t Do’s Red colour)
C. Explain one ZETO RULES that affect your daily life at work and at home.
Rule no 10- DO NOT USE MOBILE PHONE/WALKIE TALKIE WHILE DRIVING, FOLLOW THE SPEED
LIMIT AND USE YOUR SEAT BELT.
D. What will happen if you do not follow ZETO rules?
Disciplinary action if fail to comply. For contractor, snon compliance of the rules will be investigated. If
confirmed not comply, written warning given, removal from site permanently/ temporary and may lead or
result to termination of contract,
E. How does ZETO rules related to your daily operation?
The relevant rules to be comply all times during operation, e.g Rule no 1, 2, 3, 5, 6, 8, 9, 10.
F. Application of ZETO Rules
Apply at all site of operation (company vessel, PCSB site e.g platform and PCSB group of companies).
Briefing, training, in house coaching, meeting conducted as best to apply rules to ensure all personnel
apply and comply with the rules,
G. Action as Master if crew breaks the rules.
Master will remind his crew, carry out briefing of the rules for compliance and might report to the
management if reoccurrence.
2. PCSB HSEMS AND COMPANY HSEMS/ BRIDGING DOCUMENT

A. How many PCSB HSE GUIDELINES are there?


17 Guidelines such as:
B. Give two examples of PCSB Guidelines that you are familiar with?
CIMMV (Charterer Instruction To Master of Marine Vessel), Fuel Bunkering Guideline,Drug and Alcohol
Guideline.
C. How many element are there in your company HSEMS?
Company has SMS (SAFETY MANAGEMENT SYSTEM) Manual. There are 12 elements as per ISM Code
Requirement.
The SMS consist of 4 volumes:
*Vol.1 –Safety Management Manual (SMM)
*Vol.2-Shipboard Operation Manual (SOM)
*Vol.3-Emergency Response Procedure (ERP)
*Vol.4-Form and Checklist (FORM)
D. What is your understanding on HSEMS?
HSEMS-Health Safety Environment Management System.( PCSB has 9 elements). HSEMS is to identify
and control the risk of health, safety and environment in a company systematically and documented, to
achieve the objective of zero accident or incident to HSE.
E. What is your bridging document and why do you need it?
A bridging document is between contractors and PCSB. Bridging document needed as an agreement for
contractors to follow and comply with the charterer’s requirement in the aspect of HSE.
F. What is the different between ISM and HSEMS?
ISM is mandatory requirement for vessel 500 GRT and above, and voluntarily for any vessel below than
500GRT in a shipping company. HSEMS is normally set out for major oil and gas companies.
G. What is DOC and SMC, Convention and Non Convention, Voluntarily and Mandatory participation?
DOC is Document of Compliance (for the company) and SMC is Safety Management System for ship.
Convention is a vessel to comply with SOLAS requirement and Non Convention is vessels not comply with
SOLAS, but can apply voluntarily, except it is mandatory as per contract requirement from the charterer.

3. PCSB MARINE HSE GUIDELINES

A. Are you familiar with CIMMV?


CIMMV is Charterer Instructions to Master of Marine Vessel. (REV 0 MARCH 2008)
B. Brief the content of CIMMV?
CIMMV content is about Master responsibility, apply of stop work, personnel transfer , PCSB focal person,
requirement of crew safety training and medical examination, navigation warning , radio communication,
safety instruction of Master (e.g.: meeting ,drills, use of PPE, maintenance), crew change requirement,
Incident Accident Reporting, requirement of vessel in port, 500m zone of approach/arrival/departure at
installation, cargo operation, carry of passenger, anchor handling, fuel transfer operation and towing.
C. Understanding with PCSB Marine Procedures/Guidelines?
Master to brief the crew of PCSB Marine Procedures/ Guidelines, including the policies.
D. Why do we have such procedures/ guidelines?
To ensure company commitment and show full compliance with PCSB requirement and ensure the safe
operation at sea.
E. Where to find the guidelines?
PCSB Sites, Company Shore Office and onboard the ship. All the guidelines must be available at PCSB
area of operation.
4. International Rules and Regulations e.g. SOLAS, MARPOL
A. What is the certificate under SOLAS AND MARPOL Convention?
*SOLAS- Safety Equipment Certificate
- Safety Radio Certificate
*MARPOL- International Air/ Oil/ Sewage Pollution Prevention Certificate. (New certificate under MARPOL
is International Energy Efficiency Certificate @ EEC, required a vessel to have Ship Energy Efficiency
Management Plan @ SEEMP)
B. Explain SOLAS/ MARPOL in general.
SOLAS Objective is to ensure safe operation at sea of human life. MARPOL Objective is to ensure the
marine environment free from pollution.
C. How many chapters are there in SOLAS?
12 CHAPTERS
D.MARPOL- Explain the procedures before discharging bilges water overboard?
The bilges cannot exceed 15 PPM to be discharge overboard through Oily Water Separator (OWS).
E. Explain your understanding on SOLAS, MARPOL, local requirements.
SOLAS 1974 means SAFETY OF LIFE AT SEA and MARPOL 1973 mean MARINE POLLUTION.
F. Explain chapter v SOLAS requirement in brief
SOLAS chapter v is about Safety of Navigation. Bridge Navigation System, AIS, International Code Of
Signal, Distress Signal, Chart and Nautical Publication, Radar, Voyage Planning Passage Planning.
G. Explain Annex 1,4 and 5 MARPOL requirements.
MARPOL ANNEX 1: Regulations for the prevention of pollution by Oil
MARPOL ANNEX 2: Regulation for the prevention of pollution by sewage from the ships
MARPOL ANNEX3: Regulation for the prevention of pollution by garbage from ships
H. What is current ILO, STCW requirements?
ILO (International Labour Convention) & STCW (Standard of Training, Certification &Watch keeping).
Current requirement under Manila Amendment 2010, crew must undergone SSA, DSD, SSO effective 1st
January 2014 and changes to Minimum Rest Hour. 10 hour rest in 24 hours period. 77 hours rest in 7
weeks. Previous rest hour is 70 hours.
5. Local rules and regulations e.g. Merchant Shipping Ordinance 1952

A. Minimum age of employment of on ships?


18 years old
B. Explain MSO 1952
MSO 1952 is an Act enacted in the Parliament of Malaysia, and came into operation of Sabah Sarawak.
(Sabah Sarawak has MSO1960)
C. Where can you find this publication?
ISM & ISPS requirement applied onboard is one of the MSO requirements, e.g.: SMS Manual. And MSN
(Malaysian Shipping Notice) issued by Marine Department for any important info to be enforce,
implemented or share to all vessels.
6. ISM / SMC and its contents.

A. What is the objective of ISM code?


The objective of the ISM code are to ensure safety at sea, prevention of human injury or loss of life, and
avoidance of damage to the environment, in particular to the marine environment and to property.
B. What is the difference between SMC & DOC?
SMC is safety management certificate, is issue by Class and the certificate mainly purpose is to certify the
ship is meet the ISM requirement of 14.4. DOCS is Document of Compliance this certificate is certifying the
company is implementing and meet the ISM requirement, the certificate is issue by Local authority or
marine department.
C. Explain ISM?
ISM is International Safety Management code, for the safe operation of ship and for pollution prevention
adopted by organization.
D. What certificate fall under ISM?
-INTERIM DOCUMENT OF COMPLIANCE
-DOCUMENT OF COMPLIANCE
-SAFETY MANAGEMENT CERTIFICATE
-INTERIM SAFETY MANAGEMENT CERTIFICATE

E. How to educate crew onboard with ISM?


-Conduct briefing and familiarization on board, Site Inspection visit and during meeting

7. Understanding on MASTER OVERRIDING AUTHORITY

A. In what situation can Master practice this authority?


Master to practice MOA if he thinks the operation is not safe to carry our due to unsafe act or unsafe
condition, such as bad weather.
B.MOC understanding, Change Over Command process onboard and what is your understanding on
Overriding Authority as a Master. What are the difference MOA, SWP and NOP?
Management of Change. Change over command between Master & Chief Officer, or Master to Chief
Engineer. Master can decline the task if he thinks the task can expose to an unacceptable risk which cause
injury to his crew, property & marine government. MOA is risk for Master which has overriding authority.
Everyone can has the right and can apply SWP. NOP is a protest statement issued by Master to other
vessel Master when accident happened at sea.
C. What/ When/ Where and Why apply?
Apply stop work. At anytime if the operation is not safe to do. At offshore during cargo operation, fuel
bunkering, passenger transfer, etc. To ensure no one get hurt & avoid accident and incident happened.
D. Difference between MOA & STOP WORK
MOA only Master can decide & have authority. STOP WORK can be applied to everyone, but to inform or
notify Master or the company.

8. Explain EMERGENCY RESPONSE PROCEDURE

A. Given a situation below (what is your action)?


i) Fire in the engine room
Sound fire alarm, notify all personnel, active fire fighting system as Co2 and ready for evacuation if
necessary.
ii) Bridge control failure
Active general alarm, notify all personal, maneuver the ship to minimize the effect as low as possible.
Ready to abandon ship if necessary.
iii) Vessel grounding
Stop engine, Sound general alarm, Notify all personnel and muster ship personnel at muster station, initial
search and check around the ship to make sure the ship no breaching. Ready to distress signal to the local
authority if necessary.
iv) MOB
Release lifebuoy light and smoke signal, sound man overboard alarm, post lookout with binoculars and
look for the crew, perform Helmsman/ Williamson turn and ready to launch rescue boat. Broadcast urgency
to ship in the vicinity and radio room.
v) Medivac
Request emergency aviation service for the MEDEVAC, Notified local authority and arrange the nearest
secure medical attention, the progress above should record in deck log book.
B. Briefly explain onboard ERT/ ERP. Reporting sequences and understanding on INF and IRF
ERP is emergency response plan @ ERT is emergency response team (shore base team/ office- GM,
DPA, HSE, TECHNICAL, OPERATION, CREWING), each of the personnel crew is assign to the specify
position and role, during emergency occur.(Muster list)
3. SOLAS requirement on drill/ drill matrix
SOLAS drill conduct regularly as per requirement, monthly, quarterly, 6 monthly, annually. ( 2 monthly for
confined space drill)

9. Rest hour period as per STCW95 of fatigue management

A .Understanding of rest hour period as per STCW 95


Seafarers must have 10 hours rest in any 24 hours, but except during emergency. Minimum rest hour in 7
days increased to 77 hours, from 70 hours (as new requirement under Manila Amendment 2010)
B .Requirement as per MLC 2006
Seafarers must have 10 hours rest in any 24 hours, which may be divided into no more than 2 periods, one
of which shall be at least 6 hours length , and not more than 14 hours
C. How to monitor Work and Rest hour onboard
Company SMS Manual has form of Work & Rest period for recording. Chief Officer will monitor and verify
by Master

10. Change of Command procedures/ process

A. What is your company procedure for taking over command?


Procedure of taking over command can be made during senior officer (Deck & Engine) handling over
process.
B. Procedure and Explain
Company SMS Manual has handling over procedure for senior officer (Master, C/Engineer & Chief Officer).
This process took place where there is sign on & sign off senior officer meet onboard, which means of the
sign on senior officer will ‘taking over command of the ship’.
11. Drill and Training requirement

A. Explain drills that need to be conducted as per SOLAS requirement


Abandoned ship drill, Rescue boat drill, Fire drill, other drill such as Emergency Steering gear drill,
Grounding drill, Collision drill, etc. New drill to be conducted is confine space drill
B. How do you conduct drill?
Drill to be conducted frequently (Monthly, 3 monthly, 6 monthly. The drill must be practical and carry out as
per real situation happened onboard. At the same time, LSA& FFE to be tested on the functionality such
as fireman outfit, BA set, fire hose/ hydrant , Stretchers, Portable fire pump and etc.
12. Note of Protest

A. What is the note of protest and when do you need to issue NOP?
Statement or document of protest to 3rd parties (other company). NOP applies when there is an incident
happened between two parties, mostly a collision between 2 vessels.
B. Understanding on Note of Protest, the reason why and when to issue NOP
Note of protest issued as an agreement between 2 parties Masters when incident happened, both must
sign the NOP.
13. MOPO

A. Under which PCSB HSE guidelines can you find MOPO?


MOPO can be found in CIMMV Rev 0 MARCH 2008
B. Understanding of MOPO. Why and when it implies
MOPO is Manual of Permitted Operation. MOPO is a guideline for Master. MOPO does not override
Master decision. Normally master use MOPO guidelines when doing passenger transfer (e.g.: no transfer if
wave > 2.5m & wind > 25knots)
C. Deviation to MOPO. What shall be taken into place?
Master can override MOPO and can stop the job after he makes decisions if he thinks it is unsafe in line
with PCSB & SOLAS requirements.
14. Personnel transfer procedure

A. What is the procedure for personnel transfer?


Briefing to be conducted before transfer, and watch the video of Safe Transfer at sea. Master must
confident & have clear view when doing transfer& observe MOPO, assist by C/O, C/E standby engine
running &maintained. Appropriate PPE to be worn .Do not carry luggage in hand during transfer & hold
both hand to the swing rope during transfer. If in doubt, stop. Assess the deck level between both vessels
and boat landing. Master to stop work if the weather is not permitted
B. Given a scenario describe how best to apply this procedure
During personnel transfer between boat to boat & boat installation or vise versa.
15. Bunkering procedure

A. What is the procedure for bunkering?


PCSB Fuel bunkering guidelines
B. Describe bunkering procedure as per MOPO requirement
Fuel bunkering can be carrying out if the wave is below 1 m. No fuel bunkering if wave more than 2.5m &
wind more than 25knots
16. 500M Zone Procedure

A. What is the procedure and what is PETRONAS Guidelines. What is the most crucial item to check?
Can be found in CIMMV. Master to ensure vessel not approach 500m from the installation, only upon prior
permission and approval from the offshore installation then vessel can enter within the zone. Master to
communicate with offshore installation to check & get advice of if there is any activities such as diving or
ROV within the area. Other activities is to check the anchor pattern, anchor mooring buoy, avoiding of
crossing anchor wire and aware of any floating vessel.
17. Stop Work Policy

A. Who, Why, When and How to comply


Everyone can apply stop work if he or she thinks the jobs is not safe.
18. Understanding of JHA/ RA/ TOOLBOX /PTW

A. Describe your understanding on assessment tools


Assessment tools are the method that uses to identify activities hazard as example JOB HAZARD
ANALYSIS, HIERARCHY CONTROL AND RISK MANAGEMENT
B. How to conduct good assessment tool?
Familiar and understanding on assessment tools
C. Objectives and review upon any assessment
To ensure the preventive action is effective and practicable
D. Understanding of behavioral safety and experience with other PCSB with safety tools
Behavior safety is analysis; study and understanding focus on people behavior, a safety partnership
between employer and employee.
19. PCSB Contractual requirements which includes Technical Specifications and Charterers instructions

A. The understanding with OSVIS Requirements. Why we have OSVIS?


OSVIS mean OFFSHORE SUPPORT VESSEL INSPECTION SYSTEM
B. Offshore duration as onboard standby vessel and milk run vessel
Standby vessel= 60 days (after 45 days must arrange crew change)
LCT = 90 days (after 75 days must arrange for crew change)
C. Requirement of PETRONAS OSP and the difference between SHELL/ EXXON/Other Pcs SAFETY
passport?
requirements of OSP are valid training in line with PCSB & STCW & valid PMU
