REF : LETTER OF CONTRACT APPOINTMENT – xxxx

With reference to your application, we are pleased to offer you the position as xxxx with effect from
xxx on the following terms and conditions:-

TERMS AND CONDITIONS OF EMPLOYMENT

1) PROBATION PERIOD

1.1) You will serve a probation period of three (3) months from the date of commencement
of employment. The company reserves the right to extend your probation for a further
period of three (3) months if necessary. On or before the expiry of your probationary
period or the extended period as the case may be, you will be confirmed in your
employment in writing, provided your performance measures up to the Management
satisfaction.

1.2) The termination notice during the probationary period will be xxxxx notice or pay in lieu
of notice in writing by either side which can be served at any time.

1.3) Whilst on probation, the employee will not be allowed to take annual leave, unless on
emergency. Any leave application before the completion of probation period requested
would be strictly at the discretion of the Management.

1.4) The termination notice upon confirmation of employment will be one (1) months’ notice
or pay in lieu of notice by either side which can be served at any time.

2) SALARY

2.1) You shall be offered monthly basic salary of RM2,300.00 (Ringgit Malaysia Two
Thousand Three Hundred) per month during probationary period.

3) TRANSPORT / PARKING / HAND PHONE ALLOWANCE

3.1) Your monthly motor vehicle allowance is RM550.00 (Ringgit Malaysia Five Hundred &
Fifty) per month for use of your own car for Company business.

3.2) Petrol Allowance - The rate of RM0.40 (Ringgit Malaysia Fourty Cents) per kilometer
will be paid as petrol allowance for use of your car for Company business. This rate
may change from time to time according to the retail price of petrol and the changes
are at the sole discretion of the Company.

3.3) Parking / Toll Fees - The Company will reimburse your parking and toll fees based on
original parking receipts and / or used parking coupons and toll receipts when you use

3.4) Your monthly hand phone allowance inclusive of data plan is RM150.00 (Ringgit
Malaysia One Hundred & Fifty) per month for use of your personal phone for Company
business.

Any allowance scheme offered is at the full discretion of the Company and may be reviewed
from time to time to suit prevailing conditions.

4) CURRENT WORKING HOURS

4.1) Total hours of work is 45 hours per week.

Days Working hours

Mondays - Fridays 0830 – 1730 hours
Saturdays 0830 – 1330 hours
Lunch 1230 – 1330 hours

5) HOLIDAYS

5.1) During your employment with the Company, you will comply with the regulations
governing Federal Public holidays and Selangor State holidays enforced by the
government in Malaysia.

6) ANNUAL PAID LEAVE ENTITLEMENT UPON CONFIRMATION OF EMPLOYMENT

6.1) During your employment with the Company, you will comply with the regulations
governing annual leave entitlement in accordance with the Malaysia Labour Law.

8 (Eight) days - After 1 year and over but less than 2 years
12 (Twelve) days - After 2 years and over but less than 5 years
16 (Sixteen) days - After 5 years and above.

Such leave must be taken at times convenient to the Company and you are required to
submit a written application four days in advance, and is subject to the Company’s
written approval.

Annual leave must be taken within the same year. Accumulation of annual leave has
to be approved by the Company, and not more than half of your total annual leave is
entitled to be accumulated.

7.1) Normal
medical
expenses from general practitioners are payable by the Company. Our
Company’s panel clinics are Klinik Ronnie / Klinik Mediviron / Klinik Union Medic / any
other non panel clinics convenient to you.

Please obtain medical cards for Klinik Ronnie and Klinik Mediviron from the HR
Department. You are required present the medical cards during each visit for billing to
Company.

You are required to obtain receipt for visiting to non-panel clinics and request the clinic
to state your illness clearly on each receipt or medical chit to be eligible to make claims
from the Company.

7.2) The Company will not bear the medical cost of the following: -

 Medical expenses from medical specialists.

 Medical expenses arising out of self-inflicted injuries or resulting from unlawful
acts.
 Expenses incurred in respect of dental or optical consultation, medical
expenses incurred arising from social diseases.

7.3) You are entitled to paid sick leave not exceeding a total of 14 working days in a
calendar year upon producing a medical certificate. Medical expenses claim of up to a
sum of RM480.00 (Ringgit Malaysia Four Hundred & Eighty) per year. However, this
medical benefit is not reimbursable if not fully utilised.

The above entitlement does not include optical treatment, dental treatment,
hospitalised fees as well as any sickness or injury which is self-inflicted by you or
which rises from your misdeed or misdemeanour.

7.4) However, the Company has in placed Group Personal Accident insurance policies
which includes hospitalisation, for confirmed staff. You may check with the
Administration Department for further details.

8) EMPLOYMENT PROVIDENT FUND (EPF), SOCSO, EIS & INCOME TAX DEDUCTION

8.1) The prevailing government regulations would be followed with respect to the above.

9) COMPANY SECRECY

9.1) All information obtained in the course of work with the Company shall be deemed to be
strictly confidential. No such information shall be divulged to any outside party.

10) COMPANY RULES

10.1) Your employment shall subject to the Company’s rules and regulations and other
procedures as may be made known to you from time to time.

It is absolutely
essential that
you will devote the whole of your time and attention to the services of the Company
and shall not employ yourself or be engaged, concerned or interested either directly or
indirectly, alone or jointly with other persons, firms or companies without prior consent
in writing from the Company.

12) NON-COMPETITION AGREEMENT

12.1) Regardless of the reason for the termination of the employment contract, an Employee
is prohibited from undertaking in any activity that competes with Mun Hean Group for a
period of two (2) years thereafter, either directly, by exploiting an undertaking himself,
or indirectly, by accepting employment with other companies in a similar industries /
competing activity.

13) OTHERS

13.1) Should you not report to work on the agreed date after signing and accepting this letter
of Appointment, the Company shall reserve the right to recover one (1) month's salary
from you, failing which legal proceedings shall commerce against you to recover the
salary amount.

Our offer is subject to your passing the Company doctor’s medical examination, if required.

Please confirm your agreement to the terms and conditions of this appointment by signing below
and returning a copy of this letter to us.

Please be aware that the offer letter must be signed and returned within one week from the date of
this letter. If we do not receive your signed acceptance within this timeframe, the offer letter will be
considered invalid
