To be completed and returned by the Participant and parent or legal guardian of all U-18s taking part in
this trip (To be handed over to the trek leader).

CODE OF CONDUCT
Participant are requested to adhere to the timing as mentioned in the itinerary. Smoking, alcohol,
and consumption of any intoxicating substances are strictly prohibited. Any deviations by the
participant after the time specified by the staff coordinator during the tour will be at one’s own
risk.
Participant are individually responsible for their baggage, valuables, and travel documents.
Participant are expected to conduct themselves in areas responsible and dignified manner all
through the trip. The decision taken by the staff coordinator on the spot on any matter will be final.
In the unlikely event of an accident or an illness during the event which needs immediate treatment,
I shall agree to me/son/daughter receiving first aid and medical treatment from qualified
practitioners, including anesthetic and blood transfusion, that may be considered necessary by a
licensed medical doctor.
I Undertake to inform the trek leader/organizer as soon as possible if there are any changes in
medical circumstances between the date signed below and the start of the event.
I hereby undertake that I have read and completely understand everything that is mentioned above
and me/son/daughter will abide by the above-mentioned code of conduct. Failing this, I shall have
no problem with the disciplinary action initiated by the organizer.
