Abstract
Community Action proposal Assignment 2

In this assignment I will be creating a proposal for a marginalized population within my

community whose mental health needs have been overlooked or ignored by service providers.

Within the assignment I will identify and break down these key areas. Name of my organization

and its location I will identify the purpose, vision, or mission statement of my organization. I will

then provide a Needs analysis and assessment. This will be followed by laying out the program’s

goals and objectives. I will list the services provided and describe how this organization will

relieve and provide for this population. I describe the structure of the organization and the staff

that will be employed. I will then provide an assessment of the population along with an

extensive budget followed by a closing paragraph addressing sustainability.

Name and Location

The name of my organization is Caring Hands Recovery. Caring Hands recovery will be

located in Johnson County Kentucky. Johnson County is located deep in the Appalachian

Mountains in the bible belt. It is a small and quiet town that has been attacked by drugs nonstop

for the past 30 years. This organization was named Caring Hands Recovery because it is my

belief that it takes caring ands to heal individuals needing recovery assistance. I chose to

establish this organization in the selected community due to the drug epidemic in the area.

Johnson county has been struggling to stay afloat in the opioid drug epidemic. When establishing

a specific organization its location is a key factor. When serving a specific demographic

population, it’s a good idea to place your organization as close to that population as you can.

This area is in much need for a recovery center that was the main reason behind selecting

the location. Its ideal to place a recovery center where there is none as it would serve the local

community and surrounding communities that may be at need. To best serve the marginalized

community, the organization will meet them more than halfway. We will meet them in the eye of

the storm rather than the outer lands. I believe that we could all use a little help from someone

who cares in life, and I want the name Caring Hands Recovery to advertise that in its name, so

our community knows we care, and we want to make a difference in their lives.

Our Vision

The vision of Caring hands recovery is to provide full spectrum care to the substance use

population. We will offer primary care along with mental and behavioral health services. Caring

Hands Recovery is a new age revolution in substance use treatment methodology. We treat the

mind and body. Our goal is to be the one stop medical provider for the recovering addict. We

will offer you counseling, prescribe your blood pressure or anxiety and depression medications

and treat your cravings with medications like suboxone or methadone.

This clinic is designed to offer top level care to those who struggle with substance use

disorder. By treating the addicts mind and body they have a better chance for recovery. We want

to send as many stable and sober individuals back to their family’s and out into the public as

possible. We will minimize stress levels and confusion of the medical system by offering

everything you need in house at our clinic.

We are confident with our staff and our numerous approaches that we can and will

change the face of recovery for the better and show that we are a contender in the world or

primary care collaboration with mental and behavioral health services. We will develop and

provide top level care to all individuals that come and seek help from us. Final we will not

discriminate race, gender, sexual orientation, or creed. Everyone is welcome at Caring Hands

Recovery.

Needs Analysis and Assessment

Our target population will be the substance use community. This is a large portion of the

total community of Johnson County (Johnson County Substance Use | LiveStories, 2021). This is

a population that is marginalized and underserved. There are no inpatient facilities in this town or

the surrounding towns, so this is a much-needed service to the community. Johnson County

Kentucky has a population of 22,634 (Johnson County, Kentucky Population 2021, 2021).

Individuals in eastern Kentucky are 55% more likely to become addicted to drugs between the

ages 12 and 23 (Kentucky, 2021).

That leaves us with a total possible chance of drug use somewhere around 12,448.7 that’s

a huge amount of possible and or active drug users in one county. Know the math and statistics

behind the chance for probability of substance use really helps to place the need for an

organization like this in perspective. Addiction in this area is like mold it will continue to grow if

it goes unchecked and untreated. One of the main issues is the target population’s location. This

area is covered with hills and valleys so transportation to other towns can be an issue during rain

or other weather events.

The organization will help address the presenting problems of the substance use

community by offering a full spectrum of treatment options to them. Caring Hands Recovery

will offer primary care for those with issues such as hypertension, diabetes and drug related

issues like Hepatitis C and HIV. We will also provide individual and group psychotherapy for

mental health issues and substance abuse counseling utilizing 12 step methods as well (The 12

Steps, 2021). Part of the treatment will include case management to better help the patient adjust

back to a normal life once they have completed treatment with us or while the are working their

way out the door. We hope to offer training programs for individuals wishing to go back to

school for education related concerns such as GED or collage prep for those that have dropped

out of high school or never received an education.

Another great service that our case management team will offer is budgetary training for

the patients. A huge part of returning to normalcy and becoming a productive part of society is

having a job and paying bills. Our case management team will educate the patients on the

importance of a budget and how to form one and pay bills on time. Money is not an object for an

addict it’s a source of life that they use to stay alive. Once becoming sober the value changes and

it becomes a source to start a new and more stable life for them and their family so that’s what

makes budgetary education a valuable service.

As stated, the major problem is there’s no inpatient treatment centers for this community

and this organization aims to change that. By implementing an inpatient detox, counseling

services center, case management and educational class’s along with the primary care we will be

serving the population in all aspects of the treatment they are lacking in this community to

strength the marginalized population and save their life’s.

Program Goal and Objective

Caring Hands Recover will aim to accomplish the short-term goal of being fully staffed

and opened in the next 6 months. As for a long-term goal in the next 2 years we want to see the

overdose and death rate in Johnson County drop by 10%. This is a very important goal for us. By

seeing the overdose and death rate drop we know we are doing our job. Not only is treating

addiction our priority but preventing further addiction within the community and preventing

overdose related death is a key goal. We have a last of objectives we would like to accomplish

with this organization they will be listed below.

Goal 1. We want to be fully billable with Kentucky Medicaid and Medicare along with

major commercial insurance providers. We want treatment to be in sight of anyone and everyone

no matter monetary or economic status. Within the first year of operation.

Goal 2. Have every patient set up with primary care services and behavioral health

service upon discharge of the inpatient program so they may continue treatment with us here as

outpatient or a facility close to there home. Within the first 6 months of operation.

Goal 3. Implement a full-service case management team to help patient find jobs and

education opportunity’s while in treatment and after as well. We would like to see this up and

running in the first physical year of operation.

Goal 4. Provide every patient that comes through our door with appropriate and

affordable treatment we will measure this with annual and six-month surveys to ensure patient

satisfaction.

These goals are very realistic for this area and by meeting these specific goals and

objectives we would be doing a great service to the marginalized substance use population of

Johnson County Kentucky. The goals and objectives are set in a way to show not only growth in

the organization and what they are doing but also in the patients seeking our services as well. It’s

a huge step for someone to get a job or go back to school for an education that’s currently

working on recovery steps in their life. This would show vast growth in patient demographic

population.

Services Provided

This organization will provide a large arsenal for the substance use population to utilize

wen fighting their addiction and the needs they encounter during the struggle. Caring Hands

Recovery will provide individual substance abuse counseling using a biopsychosocial model of

treatment. We will also provide group and couples counseling to better strength the family unit

and build a support group for the patient. We will provide cause management to all patients so

that we can meet the social, educational, and economic needs of the patient thus improving

recovery. The next service provided will be primary health care. We want to service as the

primary physician for the patients we serve. We will provide physicals and annual CBC and

HMP labs to render the appropriate services and test for HIV and HEP-C (Drug addiction

(substance use disorder) - Diagnosis and treatment - Mayo Clinic, 2021).

Not only would we provide the primary care for the patients but also medicating assisted

treatment as well. If the patients would like to enroll in the MAT program, we will provide

methadone and suboxone treatment to the patients with dangerous withdraw sings and cravings.

The next service to be provided will be Mental health counseling for those struggling with

depression, anxiety, and other psychological issues that are co-occurring with the patient

substance use disorder. We will provide patient with appropriate referrals for any illness or

health related issues that fall outside of our scope of practice.

A huge part of successful recovery is returning home and not relapsing. We will also be

offering an aftercare program where the patient may call a 24-hour crisis line free of charge

where they can receive crisis, suicide prevention and referral counseling even after they have

finished treatment in our inpatient and outpatient programs.

The final service we will offer will be STD screening and counseling services for those

affected. Many addicts contract STD’s and STI’s during active addiction and once they become

sober want treatment for them and counseling for the more serious once like HIV. This is a

service we will be happy to provide for the. It is important that we treat the physical, mental,

emotional, and social issues of the patients that will be admitted to the Caring Hands Recovery

Center.

Structure of the organization.

Regarding screening for the organization to determine patient admission we will use a

biopsychosocial assessment as our intake paperwork along with collecting to forms of ID end

emergency contact info. To meet criteria to be admitted an individual must be diagnosed with

opioid dependance uncomplicated for the DSM V F11.20. Our intake counselors will make the

diagnosis on admission (Diagnostic and statistical manual of mental disorders, 2013). Patients

will be drug screened upon arrival and check-in once they sign consents to treat. We will have a

billing coordinate on site to collect insurance information and handle approvals and denial clams.

Key members of our team will include myself as COO chief operating office, 1 program

director and 1 medical director that is a medical doctor to enforce compliance with state

regulations and hand public relations with their respected departments. We will have a team of

10 counselors that specialize in mental health and substance abuse. This team will be led by a

counseling supervisor of full licensure such as LCADC and LPCC-S. The next team members

will be 5 case managers and 5 social workers headed by a LCSW. Finally, the last clinical team

members will be 3 clinical psychologists. The medical team will be made up of 3 medical

assistants, 1 RN nurse and 2 LPNs, and lead by 3 medical professionals with specializations in

family medicine APRN, psychiatry PA, and addictionology MD.

The final set of team members will be the administration support team. This will include

the front desk staff make up of 4 team members, 3 billing specialist and one office manager that

will be responsible for management of all non-medical and non-clinical staff members.

We will allow volunteers to work the phones and train in billings or assist in record keeping.

Followed up by a team of 5-10 interns. These will be students needing to fulfill their practicums

within the field. This is a massive team expected to be around 45 members.

The types of testing for this organization will included the BAM Brief Addiction Monitor

(Measures and (BAM), 2021). The biopsychosocial assessment will be the key item here that

will help us build the ITP individualized treatment plan. We will accompany the ITP with a MPI

master problem index that will list the goals of the ITP so they may be tracked for progress. We

will use the suicide prevention and risk assessment tool from the University of Columbia (About

C-SSRS | Columbia University | Child Psychiatry, 2021).

As for record keeping and documentation, we will use the online SAMMS software for

patient charts, appointment calendars and utilized the lighthouse central registry for insurance

clams and eligibility. We will utilize Microsoft outlook for our daily need such as email. We will

use word, excel, and PowerPoint in the office on all computers. Clinical notes will be placed in

charts in DAP format and case management notes will be SMART format.

Budget

The grant will be split up in many ways. We will set aside a large portion for salaries

until we start bringing in insurance money. We will need to purchase a building, medical

supplies like beds, vital monitors and contract a pharmacy for the methadone and suboxone for

the patients. It maybe be that we operate on a diminished team until we are full of patients, but I

foresee that not being an issue in this area. Outlined below will be a full budget. The $4 million

delegation will be as planned in the following yearly salary.

This will leave me with roughly another 2million left for the building and medical

supplies and the monthly bills. The clinic will stay afloat because we will bill insurance. All

counselors are billable and so are the medical physicians. I know this will work as I have

modeled this after my own office, I currently work out the only difference is this will be

outpatient and in-patient services both. My clinic is about half this size and we gross 1,809,000

last year and or staff is about 10 team members shorter than this projection, so this goal is within

grasps.

Closing

In closing Caring Hands Recovery is a great organization trying to do great things. Future

sustainability will come from the insurance companies. Every clinician can bill insurance so this

will pay their salaries. We will also take cash pay and credit care pay as well. In the future we

will also investigate new grants for working in an impoverished area. Luke 1:37

For nothing will be impossible with God (The Holy Bible, 2000). With Gods grace and guidance

this organization will turn from a dream to a reality.
