PERFORMANCE CONTRACT

S.Y. 2023- 2024

This Performance Contract is made and entered into as of September 2023, by and between
JUAN DELA CRUZ, TEACHER __ , having its school at Simeon Ayuda Elementary School, San
Vicente, Liloan, Cebu for Department of Education.

RECITALS

Department of Education desires to contract with JUAN DELA


CRUZ for the discharge of his/her duties and responsibilities as Teacher __ all as set forth
herein.

JUAN DELE CRUZ, SIMEON AYUDA ELEMENTARY SCHOOL and DepEd LILOAN
all acknowledge and agree that the purpose of this Contract is to achieve outstanding results
contemplated by this Contract to the benefit of DepEd clienteles and all agree to cooperate to
achieve the purpose of this Contract.

NOW, THEREFORE, the parties agree as follows:

I - INSTRUCTIONAL COMPETENCE
A. Lesson Planning and Delivery

1. Formulates/adopts objectives of lesson plan that are:


a. specific
b. measurable
c. attainable
d. realistic
e. timely

2.Selects content and prepares appropriate instructional


materials/ teaching aids that are:
a. adequate
b. congruent
c. appropriate
d. relevant

e. made of indigenous materials


3.Selects teaching methods/strategies that ;
a. are suitable to learning ability of pupils/students
b. provide opportunity for greater participation of the pupils/students
c. are varied and appropriate
d. maximize utilization of available audio-visual materials; and
e. generate teacher-learner/learner-teacher interaction

4.Relates new lesson with previous knowledge/skills by;
a. conducting a drill
b. conducting a review/recall of previous lessons/knowledge
c. including question from the past and present lessons

d. using informal assessment of learning

5.Provides appropriate motivation by providing;


a. using appropriate audio-visual/instructional

materials b, utilizing varied activities


c. enhancing pupils/students participation

d. relating past and present lessons

e. sustaining interest of learners


6. Presents and develops lessons that are;
a. according to objectives
b. using appropriate strategies
c. utilizing varied teaching materials
d. by relating previous lessons to present subject matter
e. according to the needs of the class.
7. Conveys ideas clearly by:

a. using language/vocabulary within the level of pupils


b. unlocking difficult/new vocabulary before the lesson proper
c. using varied examples and strategies
d. turning pupils responsive to questions asked

e. causing pupils to use knowledge learned in other situations

8.Utilizes the art of questioning to develop higher level of thinking which


is;
f. clear and simple

g. focused on application /synthesis/generalization

h. related to previous and present lessons

i. varied in type and level; and

j. directed towards the attainment of objectives

9.Ensures students' participation so that;


a.there will be interactive involvement of learners
b. conclusion of the lesson will be student generated
c.teacher will be acting as facilitator
d. teacher-stdent interaction will be generated.
e. individual and or group learning will be mobilized.

Addresses individual differences by;
a.using teaching materials/aids according to level of learners
b. providing differentiated activities
c. suiting activities/exercises to learners' ability
d.using language that is clear and simple
e.conducting remedial teaching

a. Shows manifestation of mastery of the subject matter via;


a. varied examples
b. varying levels of questions

d. varied materials and strategies
e. supplementary knowledge from textbook

b. Assessment
1. Diagnoses learner's needs using;
a. questionaire on the background of the learners
b. anecdotal records
c. previous years' academic records
d. pretest
e. analysis of data generated from a to d
2. Evaluates learning outcomes thru:
a. summative measure
b. formative measure
c. informal assessment
d. recitation; and
e. on the spot questions/tests
3. Assesses lessons to determine desired outcomes with the allotted time with:
a. at least 85% passed the summative test
b. at least 75% passed the formative test

c. Management of Time and Learning Environment


1. Maintains clean and orderly classroom all the time
2.Maintains classroom conducive to learning by way
a. arranging desks/chairs tables according to activity
b. displaying the appropriate learning materials in the corresponding subject area corner

c. making books/magazines/apparatus and related learning materials accessible anytime


d. having classroom/facilities clean and orderly
e. observing discipline and proper behaviour of pupils

10. TECHNICAL ASSISTANCE


1. Provides assistance to teachers in improving their teaching competence in:
a. development of a comprehensive lesson plan
b. selection of appropriate subject matter;
c. selection of appropriate teaching method/techniques/strategies;
d. formulation of questions
e. construction of assessment tool ( formative/summative)

b. Prepares prototype instructional materials for use like:
a. improvised equipment/apparatus
b. at least two (2) samples of detailed lesson plans with corresponding teaching
devices/summative test items with table of
specifications
c. sets of exercises/charts/pictures/posters

d. supplementary teaching devices for teachers

e. a sample of assessment instrument

c. Assists in the conduct of in-service trainings/conferences in/on:


a. preparation of training design
b. preparation of session guides
c. how to conduct training sessions
d. preparation of evaluation/training conferences
e. inclusion of new trends and development

d. Serves as demonstration teacher on innovative teaching techniques, classroom


management, materials development in region, division, district and in own
school.
11. LEARNERS' ACHIEVEMENT
a. Improves Learners’ Achievement level in the post test by 5% over pretest

12. SCHOOL , HOME AND COMMUNITY INVOLVEMENT

a. Organizes and maintains functional Homeroom PTCA:


a. as a venue to report student progress periodically
b. to serve as a link between the school and community-based projects
c. as a means to disseminate school policies/plans/programs/accomplishments
d. in order to identify areas where parents can help in upgrading instruction
e. to establish and maintain harmony of relationship between the school and the home

b. Conducts Homeroom PTCA meetings to report learners' progress at least once a month;

c. Disseminates school policies/plans/programs/accomplishments to 81% - 100% of schools


clientele;

d. Participates effectively in community projects as


chairman/co-chairman/member in community projects and in civic
organizations involving:
a. initiation of projects

b. defining and operationalizing objectives of the project

c. delineating functions and roles of persons concerned

d. identifying resources as support to the project

e. Encourages 81% - 100% involvement of schools' clientele / parents in school


programs and Activities

III – PUNCTUALITY AND ATTENDANCE


Commits to be punctual always in all dealings with DepEd including meetings & all
required school activities and have a 100% attendance and ought not to incur more than 4 days
of absence from station.
