EVALUATION PROCEDURES:

DATE PROCEDURES
On the first day, the evaluation commenced
January 15, 2024 with the presentation of the consent letter,
outlining the purpose and details of the
1st Session with Precious assessment. This transparent communication
aimed to ensure the client’s understanding
and voluntary participation in evaluation
process.

Following the consent procedure, Mental


Status Examination (MSE) was performed to
established rapport and gather initial
observations.

Subsequently, question from the (KSADS-


PL) were incorporated into the interview. The
client openness and willingness to engage
were evident as she responded to all
questions, then I gave her Personality
Inventory for DSM-5 Brief Form (PID5-BF).
While discussing the upcoming tests, the
client acknowledged a lack of understanding
in English and willingly accepted assistance
to comprehend the questions. However, the
client’s cooperative attitude suggests a
motivation to engage in the assessment

process, emphasizing a willingness to


participate despite potential difficulties.

January 18, 2024 During the second session, Precious greeted


me with big smile, indicating her positive
2nd Session with Precious engagement. I inquired about her well-being
in the past few days, initiating a casual
conversation to build rapport. We talked
briefly before we proceeding to the Personal
Emotion Index (EPI), the second test in our
sessions. I informed her that this would be our
last session and expressed gratitude for her
exceptional cooperation and willingness to
participate in this assessment.

BEHAVIOURAL OBSERVATIONS
During the initial phase of the interview, the client expressed a preference for conversing in
Tagalog, indicating a potential limited understanding of the English language. Despite this, the
client demonstrated proficient visual motor skills, maintaining consistent eye contact throughout
the interview, indicating social competence and emotional openness.

BACKGROUND INFORMATION
Precious, a 17-year-old female, one of two children of her biological parents, she has half-
siblings from both paternal and maternal sides. Her father passed away years ago, and the mother
is a single parent now that provides for them. Financial resources are strained due to gambling
behaviour, affecting the family stability. Precious exhibits signs of physical abuse by her mother
and experiences aggression from her brother, particularly when he is in a bad mood or under the
influence of alcohol. Additionally, the client faces challenges at school, reflected in low grades
and a negative impression from both classmates and teachers. The client is often involved in
trouble, which is why she frequently visits the guidance office. Despite these difficulties, the
client actively participates in sports, particularly volleyball, and is currently in a romantic
relationship.
The client’s father’s passing may contribute to a lack of familial support, potentially impacting
the client’s emotional being. The abusive dynamics within the family environment, coupled with
struggles in academic settings, suggest a complex interplay of factors affecting the client’s
mental health.

MENTAL STATUS
During the assessment, the client presented with an unkept appearance, reflecting the impact of
challenging life circumstances. Despite this, she exhibited cooperative behaviour throughout the
interview, actively engaging in the assessment process. Physically, the client appeared in good
health, with no immediate concerns noted. Her motor activity was slightly slowed, suggesting
underlying emotional distress, while her mood was assessed normal. However, her facial
expression conveyed a sense of depression, complementing her appearance. Despite
experiencing ongoing physical abuse by her mother, the client displayed a coherent thought
process and expressed no anger towards her. Notably, she articulated a future goal, of becoming
a firefighter like her father, demonstrating a positive future orientation and resilience in the face
of adversity. While her appearance and demeanour may reflect the impact of her circumstances,
the client’s cooperative attitude, coherent thought process, and future aspirations provide insight
int her coping mechanisms and motivation for personal growth. Further exploration and targeted
intervention may be beneficial to address underlying emotional concerns and support the client’s
overall well-being.

NEGATIVE AFFECT
Princess’s score of 9 indicates a moderate level of negative affectivity. It suggests that she may
experience mood swings and emotional distress more intensely than others. Given her
challenging family dynamics and academic struggles, this score aligns with potential emotional
difficulties she may be facing.
DETACHEMENT
With score of 7, Precious demonstrates a moderate level of detachment. This suggests that she
may have difficulty connecting with others and might prefer solitude. Considering the abusive
dynamics with her family and challenges in school, this detachment could be a coping
mechanism to protect herself emotionally.

ANTAGONISM
Precious’s score of 5 indicates a relatively lower level of antagonism. This suggest that she may
not be highly argumentative or uncooperative. Given the context of her family challenges, this
lower core might indicate a potential resilience in interpersonal relationships despite the
adversities she faces.

DISINHIBITION
Precious’s score of 11 suggests a higher level of inhibition. This indicates that she may engage in
more impulsive behaviors and struggle with impulse control. Considering her involvement in
trouble at school and the challenges within her family, this score aligns with potential difficulties
in regulating her actions.

PSYCHOTICISM

aspects of her life. Notably, the client faces challenges in her family dynamics, evident through a
strained relationship with her mother and ongoing physical abuse. Her close bond with her
deceased father and reliance on her boyfriend as an ally reflect a complex network of
interpersonal relationships. Academic functioning appears to be influenced by these psychosocial
factors, as the client’s involvement in fights and troubles at school, coupled with her
discontinued studies, contributes to an average grade of 81%. Despite these challenges, her
passion for volleyball serves as a coping mechanism, offering temporary relief from the stressors
in her life.

The interpretation of Precious’s personality scores suggests a complex emotional landscape


influenced by her challenging family dynamics, academics struggles and personal experiences.
Her moderate level of trust, along with signs of dyscontrol and timidity, may be attributed to the
impact of physical abuse and aggressive behavior within her family. Despite lower scores in
gregariousness, Precious exhibits a commendable effort to exert control and find stability, as
seen in her engagement in sports.

RECOMMENDED PSYCHOSOCIAL INTERVENTION

Individual Counseling
Engaging Precious in individual counseling sessions would provide a safe and confidential space
for her to explore and process her emotions, coping strategies and develop resilience in
navigating the challenges within her family and social environment.

Family Therapy
Involving Precious and her family in therapeutic interventions can address the strained
relationships, particularly with her mother and brother.
