JOINT AFFIDAVIT OF TWO DISINTERESTED PERSONS

age, both married, and residents of ____________________________ Ilocos Norte
respectively, after having been duly sworn to in accordance with law, hereby depose
and say:

1. That we personally know ABEGAIL LACAULAN GARCIA because we are


____________ and we have known her for _____ (__) years already;

2. That we personally know that her name appears as ABEGAIL L. GARICA


and that she was born on September 13, 1997 as stated on her Certificate of
Live Birth hereto attached as Annex “A”

3. That we also know that his name appears as ABEGAIL LACUALAN GARCIA
which also states that her date of birth in on September 13, 1997 on her
passport.

4. That we also know that on her employment contract is only stated as Abegail
Garcia and that it was stated therein that she was born on September 16,
1997.

5. That we attest that ABEGAIL LACUALAN GARCIA born on September 13,


1997 and ABEGAIL GARCIA born on September 16, 1997 is one and the
same person;

6. That we are executing this affidavit in order to attest to the truth of the facts
aforementioned, and for any legal purpose this may serve.
