Check if Active COI and WCB on file

Check if safety and site orientation complete


and competency evaluated

Start MR and/or PO

Discuss scope of work onsite and complete Job


hazard assessment – Use contractor form or
Bradken’s form

Issue permit to work

Contractor can start work

Enter contractor visit in the contractor log

Contractor complete the work and sign out
