AFFIDAVIT OF UNDERTAKING

I, ____________________, of legal age, ___________________________ Riverside,


Central Guisad, Baguio City after having been duly sworn to in accordance with law,
hereby depose and state that:

1. I am an applicant for Police Officer 1 and in connection therewith, I am


submitting my duly accomplished required form and supporting documents;

2. I hereby attest to the best of my knowledge that all the information contained therein
are accurate and correct;

3. I also affirm and attest that I am of good moral character and has not violated
any laws of the land;

4. I hereby confirm that, as of the date of this affidavit, I have no knowledge of any
pending complaint against me before any judicial or administrative body and I
hereby undertake that, should it come to my knowledge that a case be filed
against me after having submitted my application and during the evaluation
process thereof, I will immediately inform the Philippine National Police Office
so that the latter may act accordingly which may include the cancellation of my
application at its sole discretion;

5. I have read and will abide by the rules, regulations and requirements governing
the above-mentioned application;

6. I am aware that any willful misrepresentation of facts stated herein can be used as
basis for my disqualification; and

7. I am executing this affidavit to attest the truth, veracity and validity of all the foregoing and
to certify, under oath, the authenticity of my records, under pain of perjury.
