Influencing Health and Safety Culture

Question: What are the negative indicators of safety and Health culture at bakery?
Answer: There are many negative indicators of health and safety culture at bakery which causes the death of two
workers working in the manufacturing industry:
1. Lack of Full-time safety Advisor:
The organization had no believe in safe health and safety culture so they don’t have any full time safety advisor.
This indicates the negative aspects of the organization with low priority for safety.
2. Lack of Sufficient budget:
The organization is not willing to invest in the health and safety department considering there is no need of
safety as accidents are not happening daily. It indicates that the organization clear profit over safety mentality.
3. No safety Representative:
There was no health and safety representative in the organization. There was no one whom with workers raises
the specific safety issues. The absence of safety representative indicate the negative culture of health and safety.
4. Lack of proper Training:
There was lack of proper training and development in the organization especially in the field of health and
safety, in the first aid and emergency procedures. Lack of training increasing the risk of accidents and injuries.
5. Out dated Risk Assessment:
The risk assessment for machinery seems to be out dated. No one did risk assessment in the organization
indicate a failure to review and timely update the safety measures.
6. Insufficient Records:
There was lack of sufficient accidents and near misses’ records, the shift manager talked about the accidents are
not occurring regularly and there were no formal reports are kept for any incidents.
7. Record of Inspections:
The workers said that there was no system of check and balance as the inspections and maintenance was no
done on time. There was no system of managing the inspection and maintenance record which increases the risk
of failure equipment’s and accidents.
8. Pressure to meet production targets:
The organization put a lot of pressure on the staff and also the workers to completed their targets at any cost.
The directors also allows the workers to save the money which lead to unsafe practice and shortcuts.
9. Individually Blames:
The workers were blame individually if the targets of the organization were not met according to timeline. It
indicates the negative culture of demoralized the workers and create a fear-based culture.
10. Unqualified Personnel staff:
There was no senior maintenance engineer available at night shift. To maintain the machinery newly appointed
manager and young worker assigned the work to retrieve the falling racking themselves. Both were un
experienced and unqualified which cause the death of both.
11. Unfamiliar with specialized tools:
The lack of senior maintenance engineer allows two young workers to do the task who were totally unfamiliar
with the specialized tools to open the side panels of the oven.
12. Improper Temperature Monitoring:
No one can check the temperature of the oven before the workers entered, because no safety advisor was
available at that time which leads to the serious injury and severe burn.
13. Lack of Leadership:
There was proper lack of leadership because no one was willing to take the responsibility of the accident and no
one available to take charge during emergency situation which indicates the condition of confusion.
14. High turnover rate:
Safety was first priority of every worker. The worker turnover rate was very high in the organization due to
unsafe workplace and negative safety culture.
15. Lack of Communication:
There was clear gap of communication and coordination between the worker and the manager during the
emergency which causes problem in the rescue efforsts and increase the severity of accidents and injury.
