I hereby submit this letter to your office requesting for a field placement letter to Nakifuma health
center III, Nakifuma sub-county for a period of 6 weeks (on 24th June 2024 to 5th August 2024) under
the supervision of a health inspector and facility in-charge.

I am a student at Uganda institute of Allied Health and Management Sciences- Mulago doing a
Certificate in Environmental Health Sciences and a resident of Mukono district.
