REQUEST FOR APPROVAL FOR SERVICING/MAINTENANCE OF ONE NASCP OFFICIAL


VEHICLE TO SUPPORT IMPLEMENTATION OF GC 7 GRANT

The 2024 GC7 grant supports NASCP in complementing Government’s effort in the HIV response.
Allocations have been made for various office running costs, including servicing and
maintenance of project vehicles.

To ensure the optimal performance of our project vehicles and prevent potential breakdowns, I
kindly request approval for the servicing of the Toyota Hilux with registration number NACA-
01Z.163FG.

Attached is the assessment request from the driver detailing the required service. I submit this
request for your kind consideration and approval.
