Introduction
This Report presents the safety performance of the aviation industry1 involved in both upstream
and downstream operations reported by participating IOGP Member Companies for 2013-2023. It is
published as part of the IOGP data series.2
The data presented have been collected specifically from Member Companies of the International
Association of Oil and Gas Producers (IOGP).
The principal purposes of this Report are to record the safety performance of helicopter and
aeroplane operations of the participating Member Companies in the years 2013-2023, and to provide
data that can be used by IOGP Member Companies and others to benchmark their performance.
The key indicators for analysis of aviation operations and safety performance are:
• numbers of fatalities
• accidents recorded
• hours flown
• passengers carried
• flight stages performed
This Report presents overall (worldwide) and regional results against these indicators, and the rates
of exposure by aircraft activity performed and type of aircraft flown.
Support from the IOGP Membership for this safety and operations review is not only appreciated, but
also vital in establishing a meaningful Report. Continued support is encouraged for the future.
It is anticipated that by sharing this information with all operators, improved Member participation
in the annual review will be encouraged, and safety initiatives can then be focused on reducing
accidents where trends are apparent.
The Aviation Subcommittee also monitors publicly available accidents from non-IOGP companies in
the oil and gas industry to maximize learning opportunities.

Database dimensions
The Report is based on submissions from 30 participating IOGP Member Companies and covers
operations in 52 countries for 2023.
Figure 1 shows the number of participating companies and countries represented for each reporting year.
Countries and companies that are subject to international sanctions are not featured in this Report
for 2022 and 2023 data.

Central 2022 collided against the water about 900 metres from a
America oil rig after an inflight loss of control on the final
approach to the helideck. After the impact the
helicopter assumed an inverted position. The 11
passengers and the co-pilot managed to escape
the inverted helicopter. The captain, although
rescued by the co-pilot after his absence on the
surface was noticed, did not survive.

2019 to Port Said, a Bell 412 helicopter, operated
on behalf of an oil field service company,
performed an emergency landing (ditching)
into the Mediterranean Sea under unknown
circumstances. Twenty minutes after takeoff from
a crane ship, the aircraft had been noted to be in
a ‘frozen state’ (no variation in altitude and speed
equal to 22kts/40km/h) on the flight tracking
system. Another Bell 412 helicopter heading on
the same route tried to establish communication,
but there was no response. Minutes later a third
aircraft (AW139) that was in the vicinity joined the
effort but when failure to establish communication
persisted, an emergency case was declared,
the emergency plan was activated in Cairo
headquarters, and an emergency team assembled.
A second AW139 was dispatched to help in the
research and after a few minutes, the crew was
finally able to observe an oil spill on the water
and beside it there were 5 persons (2 pilots and 3
passengers) floating by means of life vests in water.
After 40 minutes of searching, the company’s
pilots were able to locate the crew and all
passengers wearing the helicopter’s life jackets
65 miles north of Port Said. Immediately, the
crew made a general call with new coordinates
of the located persons in water.

passengers exited the helicopter into the life raft
and were subsequently rescued. The helicopter
rolled in the water resulting in total salt-water
immersion.

2016 North USA 07 April 2016 Offshore Part 29 2 7 0 0 0 0 Major During a business flight over the Gulf of Mexico,
America the crew felt a slight vibration of the tail rotor
with no other anomalies noted and landed
the helicopter uneventfully at their planned
destination. A postflight inspection revealed
that one of the tail rotor blade erosion shield
extensions, also known as a lightning strip (LS),
was missing and that only its side tabs remained
attached to the blade. One of the main rotor
blade tips sustained substantial damage when
the LS separated from the tail rotor blade and
impacted the bottom of the main rotor blade.
2015 Asia/ Indonesia 21 March Offshore Medium 3 5 5 0 2 0 Major The helicopter crashed in a mangrove swamp.
Australasia 2015 Twin The two pilots received serious injuries and five
passengers received minor injuries. The fuselage
came to rest inverted.
Contracted on a mission to inspect pipelines
in the GTS-4 (CPU / Central Processing Unit)
Anggana area, operating at 500ft.
The investigators found that loosening of the jam
nut of the forward main rotor servo push rod was
undetected during the daily operation and led
to the rod end bearing was not engage properly
against the push rod. The condition of rod end
bearing did not properly engage against the push
rod resulted in the asymmetric deformation of
the internal thread in the push rod during the
operation. Continuous operation with such a
condition led to deterioration of the engagement
of the forward push rod and the rod end bearing
which subsequently the rod end bearing
separated from the push rod. This resulted in
the un-commanded servo movement to the
main rotor control that led the helicopter enter
un-commanded attitude an uncontrollable main
rotor and loss of control of the helicopter

Australasia Twin While approaching platform helicopter slowed,
entered into a ring vortex state and settled into
the water. Helicopter lifted from the water and
flew 7 NM to land at a different platform.

2013 Engine approach was not monitored effectively during
the late stage of the procedure. It allowed the
helicopter to enter a critical low energy state
from which recovery was not possible (vortex). It
resulted in 4 fatalities and 4 serious injuries.

occupants evacuated the helicopter without
external aid and were rescued. The accident
is still under investigation by the national
authorities.

2022 Africa Congo 19 Passenger Twin 3 16 0 0 0 0 Major A BEECH 1900C suffered an accident just after
September movement Turboprop it landed at the airport. After touchdown, during
2022 the rolling phase, the right-hand landing gear
collapsed, resulting in contact of the airframe,
the engine cowling, the propeller, and the wing
with the ground. This flight was carried out under
a joint charter contract between 3 IOGP/ASC
members.
2020 North USA 21 May 2020 Pipeline Single 2 0 0 0 0 0 Minor A Cessna 172S struck a threshold light at around
America and other Piston 2:26 Central Time during its sole planned landing
surveillance for the day. The flight was crewed by a Captain
and an Observer. No one was injured during the
landing, but the horizontal stabilizer received
significant damage.
2018 Africa Nigeria 25 January Passenger Jet 3 4 0 0 0 0 Total The right main landing gear collapsed and
2018 movement loss tore away. Aircraft came to rest on the side
of the runway. The NAIB has the lead in the
investigation. Although still awaiting the outcome
of the technical investigation by Gulfstream and
the FDR data to make useful conclusions.
2017 North USA 26 February Passenger Twin 2 4 4 0 2 0 Major Pilot of Cessna Conquest (CE441) conducting a
America 2017 movement Turboprop passenger carry flight attempted takeoff with a
quartering tailwind. As Pilot advanced power
to engines the aircraft drifted from the runway
centerline. Within a few seconds the Pilot realized
the drift and applied reverse thrust and braking
to decelerate the aircraft in an attempt to abort
the takeoff. The right main landing gear made
contact with a snowdrift. The aircraft turned 90
degrees from the takeoff direction and came to
rest on packed snow on the runway edge. Damage
occurred to the engine cowlings and propellers.
No injuries occurred during this event.

2015 North USA 06 September Pipeline Twin 3 0 0 0 2 0 Total Following a straight in landing approach to
America 2015 and other Turboprop loss runway 05 at North Raleigh, as the aircraft flared
surveillance approximately 30 feet above the runway threshold,
it experienced what was reported as “a malfunction
on the right engine” (Note: This was subsequently
clarified as a failure of the right engine or propeller
control system). The malfunction caused the
aircraft to unexpectedly yaw to the right, with a
corresponding increase in drag, which caused the
nose to abruptly drop. The aircraft could not be
controlled with the flight controls and after briefly
touching down, departed the runway impacting the
tree line on the right side of the runway. The PIC
sustained head lacerations and was taken to the
hospital, treated and released. The SIC sustained
compound fractures of his left leg. He was taken
to the hospital and admitted. The camera operator
was checked out on site by Emergency Medical
Services (EMS) personnel and released.

Accident
For the purposes of consistency across different national regulatory authorities and to enable
accurate statistics to be compiled, the following definition of accident, taken from ICAO, will be
assumed for all IOGP reporting purposes:
Accident - An occurrence associated with the operation of an aircraft which, in the case of a
manned aircraft, takes place between the time any person boards the aircraft with the intention of
flight until such time as all such persons have disembarked or, in the case of an unmanned aircraft,
takes place between the time the aircraft is ready to move with the purpose of flight until such time
as it comes to rest at the end of the flight and the primary propulsion system is shut down, in which:
a) a person is fatally or seriously injured as a result of:
1) being in the aircraft, or
2) direct contact with any part of the aircraft, including parts which have become detached
from the aircraft, or
3) direct exposure to jet blast or rotor downwash, except when the injuries are from natural
causes, self-inflicted or inflicted by other persons, or when the injuries are to stowaways
hiding outside the areas normally available to the passengers and crew; or
b) the aircraft sustains damage or structural failure which:
1) adversely affects the structural strength, performance or flight characteristics of the
aircraft, and
2) would normally require major repair or replacement of the affected component,
3) except for engine failure or damage, when the damage is limited to a single engine,
(including its cowlings or accessories), to propellers, wing tips, antennas, probes, vanes,
tyres, brakes, wheels, fairings, panels, landing gear doors, windscreens, the aircraft skin
(such as small dents or puncture holes), or for minor damages to main rotor blades, tail
rotor blades, landing gear, and those resulting from hail or bird strike (including holes in
the radome); or
c) the aircraft is missing or is completely inaccessible
Because the ICAO definition is aimed primarily at fixed-wing flights terminating on land, the
following is added for clarity over the particular area of ditching or water landing:

“Where the aircraft intentionally or unintentionally ditches or lands on water as the result of
a mechanical/system failure or aircrew error, and the aircraft cannot regain flight or is not
subsequently recovered without sustaining the damage detailed in b) above, it will be considered an
accident for the purposes of the IOGP aviation safety statistics.”
For example, a ditching that resulted in a subsequent, but not necessarily immediate, rollover
and/or sinking and major repair due to water damage, would be considered an accident for IOGP
statistics. A ditching where the aircraft remains upright on its floats and is recovered with only
minimal damage, including the damage due to water ingress, would not be considered an accident,
unless paragraph a) applied.
Note: The following are not considered accidents: experimental test flights, sabotage, hijacking,
terrorism, or direct military action.
Aircraft
Any machine that can derive support in the atmosphere from the reactions of the air other than the
reactions of the air against the earth’s surface.
Aeroplane
A power-driven heavier than air aircraft, deriving its lift in flight chiefly from aerodynamic reactions
on surfaces which remain fixed under given conditions of flight. (ICAO Annex I, Annex 6)
The average number of flights per aircraft for the reporting year. Calculated as:

Fatal accident
Aircraft accident occurrence involving one or more fatalities to the occupants (passengers or crew) –
see Fatal injury.
Fatal injury
An injury that results in death in the accident itself, or up to 30 days after the accident.
Fleet
Flight stage
Helicopter
A rotorcraft that primarily depends on engine driven rotors for motion. Helicopter is an aircraft sub-
category.
Hours flown
Aircraft hours are based on “block-to-block” time (i.e. from the moment the aircraft is pushed back
from the gate or starts taxiing from its parking stand for take-off to the moment it comes to a final
stop at a gate or parking stand after landing); also known as block time.
Incident
An occurrence, other than an accident, associated with the operation of an aircraft which affects or
could affect the safety of operation.

Injury
Any injury that requires medical attention beyond first aid. See Fatal injury.
Damage or failure that adversely affects the structural strength, performance, or flight characteristics
of the aircraft, and which would normally require major repair or replacement of the affected
component. Not considered in substantial damage are; engine failure or damage limited to an engine
only, bent or dented skin, damage to landing gear (to include wheels and tires), flaps, or wingtips.
Near miss
A narrowly avoided event having safety implications to all types of (aviation) operations.
Number of fatalities
The total number of crew and passengers who died as a result of an accident. See Fatal injury.
Used to specify where an incident cannot be logically classed under any other category.
Passengers carried
The number of passengers carried is obtained by counting each passenger on a particular flight
(with one flight number) once only and not repeatedly on each individual stage of that flight, with a
single exception that a passenger flying on both the international and domestic stages of the same
flight should be counted as both a domestic and an international passenger.
The average number of passengers flown per aircraft for the reporting year. Calculated as:

The average number of passengers carried per day. Assumes a 260-day year. Calculated as:

Serious Incident
Single Engine Piston aeroplanes have one piston-powered engine connected to the propeller, which
provide thrust to move the aircraft on the ground and through the air.
Stage
The aircraft is not repairable, or, if repairable, the cost of repairs exceeds 50% of the cost of the
aircraft when it was new.
Twin Engine Piston aeroplanes have two piston-powered engines connected to propellers, which
provide thrust to move the aircraft on the ground and through the air.
