Head Nursing Clinical Narrative Report
NARRATIVE REPORT
On October 4, 2024, we commenced our first clinical duty under the guidance of our clinical
instructor, Mr. Edberg De Guzman, MSN, RN, CNN. The day began with an introductory
session during which Sir Edberg provided a concise overview of Trauma Specialist Hospital,
familiarizing us with the hospital's structure and the scope of care provided.
Our orientation began with a tour of key areas within the hospital. Sir Edberg guided us
through the nursing station, various types of wards, and the dialysis unit. During our time in
the dialysis area, he explained the process of dialysis, detailing how it functions to support
patients with renal failure. He emphasized the significance of the procedure, ensuring we
understood its importance in patient care. Furthermore, Sir Edberg engaged us in a
discussion about various medications administered to patients, questioning us to gauge our
understanding of pharmaceutical treatments and their effects.
We then proceeded to the Emergency Department, where Sir Edberg demonstrated the
assembly of a laryngoscope, an essential tool for intubation, particularly in emergency
situations. This hands-on demonstration allowed us to observe and better understand the
critical role of intubation in respiratory emergencies. Before concluding the morning session,
we were administered a pre-test to assess our foundational knowledge and readiness for the
clinical environment.
In the afternoon, we were assigned to the ward, where we were given the opportunity to
engage in nurse-patient interactions. Under the supervision of the staff, we conducted vital
sign monitoring for several patients. This practical experience allowed us to apply our
theoretical knowledge to real-world patient care. After gathering vital sign data, Sir Edberg
permitted us to review the patients’ charts, guiding us through the process of plotting and
recording the data we had collected.
In summary, the first day of our clinical duty provided a comprehensive learning experience,
combining theoretical discussions, hands-on demonstrations, and direct patient care. We
gained valuable insight into the hospital's operations and strengthened our clinical skills
under the guidance of Sir Edberg. We look forward to continuing this enriching journey in the
days to come.
