COMPANY DATA PROCESSING CONSENT FORM

I, without any form of intimidation, force and/or duress, voluntarily and freely give my consent to the TBG
Shared Services, Inc. for the processing (collection, recording/storing, retrieval, use, retention and
disposal/destruction) of my personal information for application/employment, or internship, or business
transaction, consistent with applicable laws more particularly the Data Privacy Act of 2012, and its pertinent
rules and regulations, the TBG Shared Services, Inc. policies and guidelines pertinent thereto, and other
government agencies’ requirements.

I am aware that the TBG Shared Services, Inc. is equipped with CCTV cameras to ensure safety and
security of everybody and the establishment and I likewise give my consent to it for whatever personal data
said CCTV cameras could obtain from me.

I am aware of my rights in relation to the Personal Data that may be collected from me and my next of kin
and legal representative, including the right to be informed, of access, to object, for an erasure and blocking,
to rectify, to file a complaint, to damages, and to data portability.

I acknowledge that this Data Processing Consent is valid whenever deemed necessary. I can also refer to
TBG Shared Services, Inc.’s website at https://www.bistro.com.ph/ for more details on the TBG Shared
Services, Inc.’s Data Privacy Notice.

I submit that all the information I provided is accurate and complete. I understand that withholding any
relevant information, any misrepresentation of facts or misleading information given by me may be used as
ground for the filing of cases against me in accordance with the law.
