SCOPE
The Learning Contract applies to all Students and Professors of the College of
Nursing. They are collectively referred to as “Members” of the College of Nursing.
The Learners:
1. Proof of Enrollment. The learners are required to provide their respective Professors an
e-copy of their proof of enrollment or confirmed enrollment forms for the current term.
The said e-copy can be uploaded in the FB Course Group or in any learning platform
that is solely and privately used by the learners in the specified Course.
2. Learners Identity. In all learning platforms outside the Canvas LMS, the learner is
required to use their complete name in their accounts, following this format (LAST
NAME, FIRST NAME, MIDDLE INITIAL) and their actual photo to be used as
identification. The learning platforms outside the Canvas LMS include but not limited
to the FB Course Group, Messenger Group Chat, Telegram Groups, Microsoft Teams,
Google Classroom, and others.
3. General Survey. On the first week of classes, all learners are required to fill-up and
submit the survey form provided by their respective Professors. The said survey forms
are necessary to determine the learning and social environment of the student.
Information needed include the connectivity status of the learner, the available
electronic gadgets with screen (EGS)that the learner possess, learners’ contact
number, their guardian’s name and contact number. The said information are for the
Professor’s use only and will not be publicly disclosed.
4. Syllabus and Course Outline. E-copies of syllabus and course outline will be accessible
for students of a particular course/subject. It will be provided in the Canvas LMS by
their respective Professors or through other learning platforms, whichever is used as a
primary mode of delivery of instruction.
5. Flexible Online Distance Learning. All courses/subjects are delivered through a flexible
mode of instruction. Learners are therefore expected to embrace the synchronous
and asynchronous mode of delivery through the use of Canvas LMS with its integrated
applications like Office 365 and Google drive (File transfer/collaboration
applications), bigbluebutton and/or Zoom Meeting (video conferencing), Labster
(laboratory classes for Courses like Biochemistry, Microbiology, and Anatomy and
Physiology), and through the use of other learning platforms (FB Course Group,
Telegram Group, Messenger Group, Microsoft team, Google Classroom, and the like).
Asynchronous mode of delivery are conducted through the use of (but not limited
to) forum discussions in the different learning platforms, learning materials, pre-
recorded lectures, exercises or quizzes and assignments that can be access at any
time within the given timeframe. Synchronous mode of delivery on the other hand
include (but not limited to) video conferencing, podcast, real-time forum discussion,
real-time quiz or exercises, where both learners and Professors are online at the same
time. Relatedly, all Learners are expected to attend and participate in all learning
sessions or activities as instructed and given by the Professors.

by the learner, it is expected that the Learner informs their Professors so that learning
materials can be provided through other learning platforms used by the Professors.
Relatedly, Learners must not share learning materials to individuals outside of the
Course.
7. Conduct. Learners must observe at all times, proper decorum and netiquette. Any
misconduct inside the learning platforms will be subjected to subsequent disciplinary
actions.
8. Academic Integrity. All Learners must exercise honesty at all times. Any form of
cheating in any of the Learner’s submissions (requirements, assignments, quizzes
and/or examinations) will receive corresponding deductions or disciplinary actions,
whichever is applicable based on the University and College Policies on
dishonesty/academic integrity.
The Professors. All Professors are expected to make sure that all the provisions herein
stipulated are implemented and complied to with strict observance. They are expected
to ensure that all their students can comply by providing their learners all the necessary
assistance, aid, or support which can come in the form of:
(1) Monitoring that all the required forms, documents, requirements and assignments
are submitted by their learners on time;
(2) Providing their Learners all the necessary learning materials in order for the Learners
to comprehend the principles and concepts of the Course, apply the necessary
skills, and develop and hone the expected competence of the Course;
(3) Meeting their Learners regularly to expound, clarify, and supplement on the different
topics or subjects in the Course; and
(4) Understanding the Learners’ social and learning environment and extend extra
assistance if the situation warrants. This may include providing additional or
individualized/customized requirements or substitute activities to the Learner.
2. Embrace technology criticism. Both the Professor and the Learner are encouraged to
be open on the real problems encountered on the delivery of flexible / blended /
online learning and find ways to incorporate the solutions into the online classes
created. The Learners therefore are expected to report all issues/problems on the use
of Canvas LMS to the Professor, who in turns report the same to the Canvas LMS
Coordinator of the College.
3. Grading System. The grading system follows the University and College computation
and percentage distribution of grades. Learners who will not submit requirements,
activity reports, assignments, or examinations on deadlines will be given an
Incomplete grade (INC}.
Notations: The professor and the learner is expected to affixed their complete name and
signature on the box provided below once they signify approval on the provisions
stipulated in this Contract. This served as binding agreement between the Professor and
the Learner.
