SECRETARY’S CERTIFICATE

I, Ma. Kristina Alexandra P. Bitong, of legal age, Filipino and with office address at 426
Banawe St. Brgy. Sto. Domingo Matalahib, Quezon City, do hereby depose and state that:

1. I am the duly elected and incumbent Corporate Secretary of LRV FOOD CORPORATION,
a corporation duly organized and existing under the laws of the Philippines with
business address at Jollibee Banawe, Banawe St. Corner Ma. Clara St., Quezon City.

2. At the Special meeting of the Board of Directors of the Corporation held on August 27,
2024 at the Corporation’s business office, the following resolutions were unanimously
approved:

“RESOLVED, that the Board of Directors of LRV FOOD CORPORATION


authorized as it hereby authorize Ma. Kristina Alexandra P. Bitong
to sign, execute and deliver and all contracts, applications, and
agreements with Intellicare, with full power and authority to do,
execute and perform any and all acts necessary and or required to
represent the Corporation in the application for an HMO, the Board
hereby ratifying and confirming all that said officers shall do or cause to
be done under by virtue of this resolution.”

3. The foregoing resolutions have not been revoked and/or superseded by a


subsequent resolution of the Board of Directors inconsistent therewith and are,
therefore, still in full force and effect.
