Database Management System using LibreOffice Base: 10 marks


I. Create a database with three tables using your full name as the database name.
1) Table 1: Patient (Use the "Create Table in Design View") (1 mark)
• Columns & Specification
 PatientID: Set as an Integer, with values starting from 123, 124, 125, etc. This
field will be the primary key (PK).
 P_Name: Set as Text (VARCHAR) with "Entry Required" set to "Yes."
 Charge: Set as Decimal with 2 decimal places. Enter values with some
charges above 500 and others below 2000.
 Tax: Set as Decimal with a default value of 100.50
• Additional Task: Enter 5 records.
2) Table 2: Details (1 mark)
• Columns & Specifications:
 PID: Set as an Integer with values starting from 123, 124, 125, etc. This field
will serve as the primary key (PK).
 Date: Set as Date with the format "25 April 2022." Ensure some dates are on
or before 2020, and others are after 2020.
 Department: Set as Text (VARCHAR), with only three allowed values:
"Cardiac," "Oncology," and "Endocrinology."
 NRI: Set as Boolean (Yes/No).
• Additional Task: Enter 5 records.
3) Relationships (1 mark)
• Establish a relationship between the Patient table (parent) and the Details table (child).
4) Queries (4 marks)
• Queries using "Use Wizard to Create Query":
 Query 1: Display P_Name and Date for entries where the Date is before the
year 2020. Name this query Before2020.
• Queries using "Create Query in Design View":
(Use suitable functions to complete queries wherever applicable)
 Query 2: Retrieve P_Name and NRI to show only non-NRI entries. Name
this query NonNRIs.
 Query 3: Count the total number of Patients. Name this query
NumberOfPatient.
 Query 4: Group the Departments and calculate the total Charge for each
department. Name this query DepartmentTotal.

5) Create a Form: (2 marks)


 Use the wizard to create a form from the Patients table.
• Specification
 Add a new record to the form (use appropriate data)
 Modify the form:
- Change the background color.
- Change the font size and style of the labels.
6) Create a Report: (1 mark)
 Use the wizard to create a report from the table Patient.

Digital Documentation (Advanced) using LibreOffice Writer: 5 marks


II. All Writer files you create in this worksheet should include your full name, class,
and division as the header.
1) Create a New Document: (5 marks)
 In a new Writer document, create an index for the following topics and sub-topics
(bulleted items listed in the box).

 The content for these topics should be typed on Page 2, Page 3, and Page 4,
respectively, while keeping the first page vacant.

Additional Task: Under each subheading, fill in random text as content (use the shortcut dt, F3).

Specifications:

• Create an index titled “Communication Skills.”


• Format the index into 2 columns.

• Add one more page and include the topic provided below in the box. Update the
index accordingly.

• Apply a light background color to the index.

III. Instruction: Complete the assignment from the worksheet provided in the server folder
named "CALC DATA". Copy the file into your individual folder before starting.
