CONTRACT

This agreement ("Compensating the Gymnastics Trainer") is made effective between


Francis Joseph R. Seballos ("Instructor") and Marnelli J. Jabile ("Coach").

1. Scope of Work

The instructor agrees to provide gymnastics instruction services for the competing
athletes of San Jose – Gymnastics Club to gymnasts enrolled in the gymnastics
program. The services include but are not limited to:

- Teaching gymnastics skills and techniques


- Construction of routine for this upcoming APAAM Meet (Antique Provincial Athletic
Association Meet)
 Individual Men Routines
 Individual Women Routines
 Mixed Pair Routines
 Trio Routine
- Ensuring safety protocols during sessions

2. Compensation

I, Dolly Rose Altejos, parent of Luther Dhale A. Fortos agrees to pay the instructor a
down payment of ₱ 1,000 before December 29th and must pay the remaining balance
of ₱ 2,500 for advanced training and construction of routines before the day of the
competition. A late fee of (₱ 500) will be charged if payment is not made after 7 days of
the competition.

3. Schedule
As of: 11/19/2023

The schedule for instruction hours will be agreed upon mutually by the Instructor and
the Coach. Changes to the schedule should be communicated and agreed upon in
advance.

As of now, mandatory training for competing athletes will commence every;


Saturday – 8:00 A.M. to 11:30 A.M.

- 1:00 P.M. to 4:00 P.M.

Sunday – 8:00 A.M. to 11:30 A.M.

- 1:00 P.M. to 4:00 P.M.

The training venue will be at the Activity Center located at San Jose De Buenavista
Municipal Hall.
*Training venue will vary depending on the availability*

4. Responsibilities

a. Instructor's Responsibilities:
- Prepare a systematic training program and proper form and technique when learning
and acquiring new skills.
- Ensure a safe and supportive learning environment.
- Report any safety concerns or incidents immediately.

b. Coach’s Responsibilities:
- Provide necessary equipment and facilities for classes.
- Address any concerns or issues raised by the instructor promptly.

5. Termination
As of: 11/19/2023

This Agreement may be terminated by either party with a 7 days' written notice to the
other party. Termination does not relieve either party of any obligation incurred before
the termination date.

6. Confidentiality

The instructor agrees to maintain the confidentiality of any proprietary information or


student records obtained during the course of employment.

7. Entire Agreement

This Agreement constitutes the entire understanding between the parties concerning
the subject matter and supersedes all prior agreements.

8. Signatures

IN WITNESS WHEREOF, the parties have executed this Agreement as of the date first
above written.
