AFFIDAVIT of UNDERTAKING

I, SHELA A. PADERES , of legal age, single/married, Filipino


citizen and a resident of , under
oath, states, THAT:

1. I am one of the newly hired teaching personnel of the


Schools Division Office of Zambales, currently assigned at
District of ;
2. Corollary to my hiring, I undertake not to engage in any
private teaching to any private or public school without applying or
complying with the requirements set forth by the Department of
Education in engaging private practice of profession;
3. I am not in any manner presently engaged to any private
school or public school from other Schools Division nor have an
existing contract with any school, private or public within or without
the Schools Division of Zambales;
4. As such, I vow under oath that I have no existing contract
with any private school or any private or government agency and if
found that I have one, I undertake to immediately waive my item in
the Schools Division of Zambales in favor of other qualified
personnel;
5. Likewise, I am not administratively charged or suspended
or dismissed from government services nor have I any pending
administrative case filed before the Civil Service, Professional
Regulation Commission or Department of Education or any
government agency exercising quasi-judicial function nor have been
found guilty thereof;
6. I also undertake not to seek for transfer of assignment
from my post within a period of three years commencing from the
date of appointment;
7. I shall not hold the Schools Division Office of Zambales or
any of its personnel from any administrative, criminal or civil
liability should I later on be found distorting the truth and that I
finally undertake that I shall voluntarily resign should any of the
statement herein be found untruthful;
8. I am executing this affidavit to attest to the truth of the
foregoing statement and for all legal intents and purposes.
