Understanding your Onboarding Forms

Welcome Onboard

This document will help you in navigating and understanding your


onboarding form.
In case this document is not sufficient and you still have doubts, feel free to contact
the onboarding team via Email.

Basic Do’s and Don’ts
●​ All fields marked with asterisk (*) are compulsory fields. Form will not move to the next
step unless and until you fill all the required fields.
●​ Make sure you click on save as draft after completing every section as the page gets
refreshed automatically. If you don’t do this you will have to fill the form all over again
as the already filled data will be lost.
●​ Make sure you fill in all the mandatory fields.
●​ If you don’t have any particular document then upload a blank page.
●​ Even after completing, if the page doesn’t go to the next page then please go to the top of
the page and check the errors.

Section 1: General Information

Section 2 : Mediclaim Details (To be filled only if you are married)

Section 3: Dependent Details

Please make sure you fill this section very carefully and don’t get confused.
●​ A dependent is someone you financially support, such as a spouse, child, or parent.
●​ If you are married then you will have to fill details of your spouse (husband/wife) or your
children and if you are unmarried then you will fill details of your parents (father/mother)
●​ First Name: Make sure the first name of the dependent is as per their Aadhar card
●​ Middle Name: As per Aadhar card. This field is not mandatory. Specify middle name
only if it is written in Aadhar.
●​ Last Name: As per Aadhar card
●​ Relation: Mention your relation with the dependant
(Mother/Father/Husband/Wife/Daughter/Son etc.)
●​ Date of Birth (DOB): Specify DOB of the dependent.
●​ Address: Mention address of your dependent (Where do they live) as per their Aadhar
card
●​ Aadhar Number: Specify Aadhar number of the dependent.
●​ Minor: Tick yes only if the dependent you mentioned is a minor (less than 18 years of
age).

You can make more than one dependent by clicking on the add button at the bottom of this
section
●​ Nominee Guardian Details:
○​ Nominee: This is the person you designate to receive something of value (assets,
property, etc.) in the event of your passing.
○​ Guardian: If your nominee is a minor (under 18 years old), a guardian is a legal
adult responsible for their care and well-being.
Nominee here is the person whom you made dependent in this section.
If your dependent is not a minor, mention the same name that you mentioned as the dependent
name.
But in case if the dependent is a minor you will have to write the name and address of the legal
guardian.
For example:​
Case 1: Age of dependent is more than 18 years.
In this case you will write only the name of the person whom you made the dependent.
Example: You made your father dependent, now as your father is not a minor you will write
name of your father in nominee guardian details
Case 1: Age of dependent is less than 18 years. (Minor)
In this case you will write both the name and address of the person who is the legal guardian.
Example: You made your son dependent and he is less than 18 years of age, in this case you will
write the name and address of the legal guardian. (It can be you or your spouse)

●​ Phone number: Write phone number of the dependent and in case of minor write phone
number of the legal guardian
●​ Nationality: Mention your nationality (Indian or the name of the country you belong to)
●​ PF percentage: Provident Fund is a retirement savings scheme where both you and your
employer contribute a percentage of your salary.​
If you are making only one person dependent then the PF percentage will be 100. Do not
put a percentage sign with 100.

If you are making more than one person a dependent then the sum of each PF percentage
of all the dependents should be 100
●​ ESIC Percentage: Write 100 if only one dependent is there and if you are making more
than one person a dependent then the sum of each ESIC percentage of all the dependents
should be 100. Do not put a percentage sign with 100.
●​ Gratuity: This is a payment you may receive from your employer upon leaving the
company after completing a certain minimum service period.​
Write 100 if only one dependent is there and if you are making more than one person a
dependent then the sum of each gratuity percentage of all the dependents should be 100.
Do not put a percentage sign with 100.

Section 4: Photograph Guidelines


●​ Do not upload selfies or screenshots of the photo
●​ The background should be plain white or off‐white.
●​ The photo should have a full face, front view and eyes open.
●​ Photo should be in color and size of 2 inches x 2 inches.
●​ The photo print should be clear and with a continuous‐tone quality. 6. Maximum file size
is 10 mb

Please note you are not supposed to add all the qualifications. You are supposed to mention only
your highest qualification (highest level of education you received as of yet)
●​ Educational Category: Select the category from the given list. It can be your 10th, 12th,
graduation or post graduation degree
●​ Education Degree: Select the degree from the given list. For Example: If your highest
education is bachelor degree then mention what was the name of your degree (BSc, BE,
BTech etc.)
●​ If you are still in college or if you are still studying please tick yes under the “I am
currently a student” heading
●​ Start Date: Mention start date of your highest qualification
●​ Completion Date: Mention end date of your highest qualification
●​ Institute: Mention the name of the institute from where you completed your highest
qualification. If the name of the institute is not in the list then click the other institute
button. Once you click this button you will have to write the name of your institute
below.
●​ Educational proof: Upload a clear picture or pdf of your educational proof. The proof
can be your degree or your marksheet. If you don’t have any proof available with you
then you can upload a blank page.
●​ You have to click on the box saying “This is my highest educational qualification”

Section 10: Details of Relative working in Girnar Pvt Ltd or
Subsidiaries

Please make sure that you fill all the details of your bank account correctly and carefully. These
details will go to payroll for the purpose of your salary.

Section 12: Work experience details

Please note that providing resignation acceptance and 3 months salary slip/bank statement is
compulsory for all the experienced candidates.
●​ Resignation acceptance letter should compulsorily include LWD (Last working date).

Please note this field is mandatory for everyone
●​ Those who have a PF number are required to compulsorily mention it here.
●​ Those who have a UAN number are also required to compulsorily mention it here
●​ Those who do not have PF number, they are required to write 10 times ‘0’ (0000000000)
●​ Those who do not have UAN number, they are required to write 12 times ‘0’
(000000000000)

Section 15: Other KYC details

Please note that this field is not mandatory. Make sure that you fill in details that are relevant to you
