INTRODUCTION

The prevention of communicable diseases is critical issue in Kisesa Ward, as these diseases is a
threat to the health and well-being of the community. In order to address this issue, a
comprehensive health project plan has been developed to implement strategies that will help to
prevent the spread of communicable diseases in the community.

EXECUTIVE SUMMARY
Prevention of communicable diseases in Kisesa Ward will help to ensure the community living
in rural areas, they are access to the Healthcare and essential education of communicable
diseases such as HIV/AIDS, cholera, Tuberculosis(TB), Influenza, measles and hepatitis.

We focus on empowering the communities the communities of Kisesa Ward by enabling to have
the best ways of avoiding the transmission of communicable diseases within their areas.

Our target will concerns with provision of essential health services education according to their
population composition such as gender, age, education status etc.

This project aims to reduce incidences of communicable diseases throgh the implementation of
various prevention strategies. These strategies include community education, vaccination
campaigns, targeted awareness program and improved sanitation practices.

We aim to improve overall health and well-being of Kisesa ward community and reduce burden
of communicable diseases.

VISION: Our vision is to create a healthier and more resilient community in Kisesa ward by
preventing the spread of communicable diseases.

MISSION: Our mission is to implement comprehensive health project plan that will educate,
empower and mobilize the community in Kisesa ward to prevent the spread of communicable
diseases.
1.0 PURPOSE OF THE PROJECT
The purpose of this health project plan is to prevent the spread of communicable diseases in rural
areas especially in Kisesa ward by implementing targeted interventions and education programs
to improve health outcomes and reduce the burden of infectious diseases.

1.1 BACKGROUND INFORMATION


Rural areass often lack access to comprehensive health care services, making them more
vulnerable to communicable diseases. Factors such as poverty, lack of education, limited
resources and inadequate infrastructure can contribute ti to the spread of infectious diseases in
the community. So we came with this project to prevent the transmission of communicable
diseases in Kisesa ward, so that to improve health outcomes and reducing healthcare costs in the
long-term.

1.2 APPROACHES TO THE PREVENTION OF COMMUNICABLE DISEASES IN


RURAL AREAS.
1.2.1 Community health education: Develop and implement educational programs to raise
awareness about the importance of hygiene and diseases prevention in rural communities. This
could include training on hand washing, sanitation practices and safe food handling.

1.2.2 Vaccination campaigns: Collaborate with local healthcare providers to organize


vaccination campaigns for preventable diseases such as cholera, tuberculosis, influenza etc. This
project will ensure that the vaccines are easily accessible and affordable for residents in rural
areas.

1.2.3. Access to healthcare: Improve access to Healthcare services by providing medicine


services and community health centers in rural areas. This will help households receive timely
medical care and prevent the spread of communicable diseases.

1.2.4. Collaboration with local stakeholders: Collaborate with local people, healthcare
providers and community organizations to implement the health project plan effectively. This
will strengthen the impact of interventions and ensure sustainability of the program in rural
areas.
2.0 OBJECTIVES OF THE PROJECT

2.1 PROJECT DELIVERABLES


At the end of the project, we look forward to seeing accomplishment of the following issues.

 Develop and distribute educational materials on communicable diseases prevention in


Kisesa, including posters, and pamphlets.
 Establishment of community health committees, and train community health committees
to promote awareness, prevention ans control of communicable diseases in Kisesa.
 Provision of resources, ensure the availability of essential resources such as vaccines,
medication and personal protective equipment to prevent the spread of communicable
diseases in Kisesa.
 Organize mobile health clinics to provide health services, screenings and vaccinations for
communicable diseases in Kisesa.
 Partner with local health authorities, government agencies and non-governmental
organizations to coordinate efforts for communicable diseases prevention and control in
Kisesa.
3.0 SCOPE OF THE PROJECT
The scope of this health project plan is to prevent and control communicable diseases in the
community of Kisesa. The project will focus in raising awareness about the transmission and
prevention of common communicable diseases such as malaria, HIV/AIDS, tuberculosis and
diarrheal diseases. By executing the following ÷

 Conducting community based health education and awareness campaigns to educate


community members on the transmission and prevention of communicable diseases.
 Distribution of essential hygiene kits, mosquito nets and other preventive measures to
vulnerable population in Kisesa.
 Regular monitoring and surveillance of communicable diseases in the community to
identify outbreaks and implement appropriate interventions.
 Collaborating with local health authorities and organizations to strengthen the healthcare
system and improve access to healthcare services in Kisesa.

3.1 RESOURCE MANAGEMENT PLAN FOR THE PREVENTION OF


COMMUNICABLE DISEASES IN KISESA.

3.1.1 Personnel
 Includes health education coordinators responsible for planning and implementing
objectives of the plan
 Community health workers

3.1.2. Materials and supplies


 Hygiene kits: including soap, hand sanitizers and other hygiene products.
 Mosquito nets: to prevent malaria transmission and protect community members from
mosquito-borne diseases.
 Educational materials: posters and flyers to communicate key messages about diseases
prevention.
3.1.3 Transportation
 Vehicles for field visits: to reach remote areas of the community for health education,
distribution of supplies and monitoring of diseases outbreak.

3.1.4 Monitoring and evaluation


 Data collection tools: to track the prevalence of communicable diseases in the community
and assess the impact of interventions

3.1.5. Financial management


 Budgeting and financial planning to ensure effective utilization of resources and
adherence to allocated budget.
 Monitoring and reporting of expenditures to track projects spending and adjust as needed
to stay within budget constraints.
