AFFIDAVIT

I, GUDDI W/O LT SH. ARUN KUMAR R/O F-74, LADO SARAI,


MEHRAULI, SOUTH DELHI, DELHI-110030 hereby solemnly affirm
and declare as under:

1.That I am citizen of India and residing at the above


said address.
2.That my husband namely Arun Kumar was expired in a
road accident took place on 17.06.2021 At about 10:30
p.m. near Hanuman Mandir Andheriya Mod Chattarpur,
New Delhi.
3.That the FIR No. 374/2021 U/S 279/304-A IPC was
registered with PS: Mehrauli, DELHI.
4.That at the time of accident my husband was working
as a Ward boy in TV Hospital, Lado Sarai, Mehrauli,
Delhi and was earning 10,000/- p.m.
5.That my Husband died leaving behind following legal
heirs:

1.GUDDI (Husband of Deceased) (Age about 46 yrs)


2.VISHAL (Son of Deceased) (Age about 23 yrs)
3.ISHANT KUMAR (Son of Deceased) (Age about 19 yrs)

6.That the deponent has spent more than Rs. 2,00,000/-


upon the funeral expenses, last rituals, attending
ceremonies and many other unnecessary losses.
7.That I/WE have not received any compensation from
DLSA Office or any other government authority or SDM
court till date.
8.That my Aadhar card no. is 5284 9452 7405
9.That it is my true & correct statement.
