Respected Sir,

With due respect it is stated that I Mr. RIASAT MEHMOOD S/O MEHBOOB ALAM
SATTI serving as Junior Computer Operator (JCO) in Punjab Emergency Service, Rescue
1122, District Headquarter Hospital Jhelum (PMO Staff), whereas, Mr. SIBGHAT ULLAH
MUJADDIDI S/O AHMAD NAWAZ SAJID, appointed as same scale and designation in
Punjab Emergency Service, Rescue 1122, District Murree.

We both are agree for mutual transfer due to some domestic issues that requires our
personal for effective resolution. We will feel much comfortable by performing duty in
requested mutual transfer districts and we can look after our old parents well.

Therefore, it is requested to please issue necessary orders for mutual transfer to


concerned districts. It will be your good honor to give us an opportunity to perform our duty in
our home district.
