Sir,

I, Kirti Kothari, Nominated Owner as per Section 76 & Section 18 of Mines Act 1952,
hereby submitting offline application for appointment of mine foreman to act as a
manager under Reg. 34(6) of MMR 1961 in our Kanvera Quartz, Feldspar & Granite
Mine having ML No. 270/2005 situated near village – Kanvera, Tehsil – Deogarh &
District – Rajsamand (Raj.). The following documents have been attached with this
application letter:

1. Form 1 Appointment of Mine Manager


2. Affidavit for acting as nominated owner
3. 34(6) manager authorization application form
4. Manager documents copy (Appointment letter, Certificates, ID Proof, Undertaking)

The documents like lease agreement, GST certificate of firm, my pan card and
aadhar card have already been submitted earlier back in 2019 to DGMS Udaipur
Region.
