AFFIDAVIT OF ARREST

I, SPO1 Katherine M. Rodriguez together with PO2 Angelo Ermita , both members of the Philippine
National Police, presently assigned at Quezon City: Police Station, do bereby-depose and state THAT:

01. That on or about 9 o'clock in the morning of February 19, 2020. I, SPO1 Katherine M. Rodriguez
received a report thru a phone call from Lorraine Joloan a Public Teacher at Lagro High School that an
alleged trouble happened at the scene.

02. That immediately after receiving the report, I together with PO2 Angelo Ermita proceeded in the
reported place and saw the person who committed the attack.

03. That upon interview on Bea alonzo she alleges that Lorraine Joloan committed Direct Assault by
continuously attacking her physically.

04. That we invited Lorraine Joloan to go with us to the Police Station to clarify things which she readily
acceded.

05. That we informed Lorraine Joloan that he being held for allegedly committing Direct Assault and
apprised him of his Constitutional Rights as stated in the Miranda Doctrine, we then bought and
endorsed the case for proper investigation.

IN TRUTH TO THESE WE HEREUNTO affixed our signatures below to the truthfulness of the foregoing.
