All Records are numbered ,labelled and list of records is


displayed (based on usage or Alphabetically)
SET IN ORDER

Patient care equipments, Drug Rack, Cupboards, Trays, Wheel


chair, Stretcher kept are labelled and easy to access
Signages are kept in an order (e.g. Ward Names > Scope of
Practice > Patient Right and Responsibilities)
High Standard of Cleaning of Cupboards, Trays, and Work
Table are employed daily.
Daily Working Status of Patient care equipments, Availability
SHINE

& Integrity of Stocks are checked daily as per SOP


Regular cleaning of Posters, Fixtures, fans, tubes, and
inspection of electrical fixtures are done
ATTRIBUTES

Staff Implements SORT, SET IN ORDER, SHINE for records,


work tables, cupboards and Regular Supervision is employed
Work/Procedure instructions/SOP is displayed safe handling of
STANDARDIZE

Patient care instruments and equipments. (i.e. 5s Signage, SOP


for Multipara Monitor, RACE/PASS for fire extinguishers,
Blood & Mercury Spill Protocols)
Standard Operative Procedures for Environmental Cleaning,
Biomedical & General waste management is established and
staffs are sensitized regularly.

5s is practiced by all employees as a daily routine

Regular Formal Training Session/Hands on Training is provided


SUSTAIN

Regular 5s Audits are done and feedbacks are given

Departments maintaining High Standards of 5s are appreciated in


regular meetings/During rounds
