**Employee: By signing this timesheet, I acknowledge that the information on the timesheet shown above is correct. I further acknowledge receipt and acceptance of general terms
and conditions of the assignment listed in the offer letter. Additionally, I certify that I have reported any accident or injuries during this pay period to the client manager and Ascendo.
**Customer Call Off: This is used when an employee hasn’t reached the minimum hours guaranteed for the week and the employee is asking for the minimum guaranteed hours.
This means the facility called off the employee's shift OR the employee was sent home by the facility. It can also be used if the employee was scheduled for their minimum
guaranteed hours for the week but the employee didn’t reach the guaranteed hours due to mandatory lunch breaks.
**Voluntary Call Off: This is used when an employee hasn't reached the minimum guaranteed hours for the week. They have called out, walked off shift or assignment, was tardy to
a shift, was sent home due to COVID exposure, left shift early, was injured or ill, was terminated, or volunteered to go home early.
**Signed timesheet is due to Ascendo Resources by Sunday 11:00 AM CST. Failure to send completed timesheet may result in delay of pay as timesheet may not be accepted.
