DOCUMENTATION OF

INFORMATION ABOUT THE

PATIENT CARE, THE PROCEDURE
AND FINAL OUTCOME
INTRODUCTION :
 Documentation is any material that provides official
information or evidence and serves as a record.

CLINICAL DOCUMENTATION:
 It is the creation of a digital or analogue record that
gives details of medical treatment, medical trial,
medical imaging or clinical tests.
HIS

PACS RIS
Hospital information system(HIS):
HIS provides,
❑ Demographic data of all the
patients whether or not they have
any imaging procedures performed.
❑ Also records the admission and
discharge dates, outpatient
appointments etc.
❑ Other modules : laboratory reports
COMPONENTS:
Include the following:
 Clinical Information System (CIS)
 Financial Information System (FIS)
 Laboratory Information System (LIS)
 Nursing Information Systems (NIS)
 Pharmacy Information System (PIS)
 Picture Archiving Communication System (PACS)
 Radiology Information System (RIS)
HIS consists of:
 Medication Management
 Electronic Medical Record
 RIS/PACS
 Financial Management
 HR Management
 Materials Management
Benefits:
 Easy Access to Patient Data
 Structured Information
 Improved Drug Prescription and Patient Safety
Radiology information system(RIS):

❑ A separate system or a part of HIS

❑ Stores information specific to the radiology
department
Functions performed by RIS:
❖ Patient registration
❖ Examination scheduling
❖ Patient tracking
❖ Film archiving
❖ Report generation
❖ Administration and billing
❖ documentation
PACS:
 PACS refer to a computer system that is used to
capture, store, distribute and then display medical
images.
 For diagnostic imaging applications, PACS technology
can be utilized to achieve near filmless operation.
Major components:
Types of PACS:
 FULL PACS:
PACS system where all imaging modalities in radiology are
connected to the PACS system, with the ultimate goal of
eliminating film.

 MINI-PACS:
PACS system limited to one modality or some other subset
of the medical imaging equipment in radiology.
IMAGE COMPRESSION:
❖ Once images are acquired and sent through the
computer network, they are generally displayed for
viewing and stored temporarily or permanently for
retrospective viewing and analysis.
❖ The digital images acquired are large files with varying
matrix size.
❖ One way to manage the size of image files for
transmission and storage is image compression.
▪ Purpose of compression is to speed up transmission
of information and to reduce storage requirements.
▪ Compression can be either :
➢Lossless or reversible compression: where no
information is lost in the process.
➢Lossy or irreversible compression: where some
information is lost in the process.
PACS and information systems integration:
❖ The role of information systems integration in radiology have
identified at least five separate information systems, including
▪ PACS
▪ RIS
▪ HIS
▪ A voice-recognition direction system
▪ Electronic teaching/research file system
❖ Integration of these systems is essential because it is intended to
solve many problems.
DICOM:
❖ Stands for ‘’ Digital imaging communication in
medicine.’’
❖ It is the imaging communication protocol for
PACS.
❖ DICOM provides standardized formats for
images, a common information model,
application service definitions and protocols for
communication.
STORAGE CAPACITY:
 The storage capacity of the different storage devices is
determined by the number of bytes that the device can hold.
 Storage capacities are expressed in kilobytes(K), MB,
gigabytes(GB), and terabytes(TB).

Main CT has a storage capacity of 1 TB

TELERADIOLOGY:
 The transmission of radiological patient images, such as X-rays, CTs,
and MRIs, from one location to another for the purposes of sharing
studies with other radiologists and physicians.
 Teleradiology improves patient care by allowing radiologists to provide
services without actually having to be at the location of the patient.
 Teleradiology utilizes standard network technologies such as
the internet, telephone lines, wide area network(WAN), local area
network (LAN)
 Through Teleradiology, images can be sent to another part of the
hospital, or to other locations around the world.
Main classes of strategies for preventing
errors and adverse events include:
❖Tools that can improve communication
❖Make knowledge more readily accessible
❖Require key pieces of information (such as the
dose of a drug)
❖Assist with calculations
❖Perform checks in real time
❖Assist with monitoring
❖Decision support system
