A) REQUIREMENTS
1) This form provides the functional test for the lighting circuits and relays. Perform one such test
per floor
B) OBJECTIVES
The project has a number of lighting circuits controlling different lighting areas. These include
fluorescent lights, compact CFLs, ascent lights, exit lights etc. Different areas are on different
circuits.
The objectives of this test are:
1) Ensure that lighting circuits are not cross circuited
2) Ensure that the lighting circuit wiring matches the drawings
3) Ensure that the exit lights are not on a manual circuit which can be turned off accidentally
4) Ensure dimmable lights (in conference rooms)

F) Prerequisites
The following items must be completed prior to performing the testing procedures:
1) Final installation of this system component, including final connection to the power supply panel–
installation verification forms accepted.
2) Equipment to be controlled is operational.
3) Wiring and design manuals and submittals check.

The following personnel should be present for the testing:
1) Owner’s witness
2) Commissioning Coordinator
3) Commissioning Authority
4) Contractor’s Representative
5) Test Authority
H) TEST EQUIPMENT
The following test equipment is required to complete this procedure:
1) Timer
I) PROCEDURE
1) Participants sign in on the log below.
2) Record the equipment data on the next table.
3) Verify the prerequisites as shown.
4) Follow the step by step instructions in each step following.
5) Attach additional pages for notes if necessary. To facilitate photocopying, do not write on the
backs of forms.

S) TESTING PROCEDURE
Perform the following steps and indicate Pass/Fail result. Describe any deviation, or elaboration, on the
test procedure in the Notes section of the record sheet. Attach a separate sheet for notes if necessary.
1) The majority of the testing will be accomplished via manual testing in the field. Please keeps that
in mind and follow the requested waiting times between certain steps to allow the test personnel to
record and respond to the changes being made.
2) In addition to waiting the appropriate times, recording the time of test steps is critical in
allowing the tester to locate the events in the logs.
3) Record the date of the test: Time:
4) Identify the manual switches and/or lighting circuits as per drawings
5) Time: turn off all lights and circuits
6) Switch on first circuit
7) Switch on manual switch
8) Identify connected lights and mark on provided lighting layout
9) Is connected light consistent with design drawings PASS/FAIL
10) Repeat the test and mark on floor plans
11) Identify cross circuited fixtures in the space provided below

21) Note: Exit lights are on full back-up
22) Restore lights to original before proceeding
