Application of Certificate of Origin (C.O.

)
Malaysian Manufactured and Processed Products

1. The applicant’s company is required to attach the following documents together with the
completed C.O. application:

(i) Manufacturer
1. Invoice
2. Packing List
3. Bill of Lading / Airway Bill
4. Customs Export Declaration Form (K2) with Official Receipt / Custom Release
Document
5. Such other documents as KLSCCCI may deem necessary

[Yearly basis, to be submitted together with other registration documents]


6. Manufacturing License / Business License
7. List of Products & Cost Analysis

OR

(ii) Trader
1. Exporter’s Invoice
2. Exporter’s Packing List
3. Bill of Lading / Airway Bill
4. Customs Export Declaration Form (K2) with Official Receipt / Custom Release
Document
5. Manufacturer’s Invoice
6. Such other documents as KLSCCCI may deem necessary

[Yearly basis, to be submitted together with other registration documents]


7. Letter of Declaration from Manufacturer, for Malaysian Origin Products
8. Manufacturer’s Manufacturing License / Business License & Company
Registration Certificate
9. Cost Analysis of Products from Manufacturer

a) All the above mentioned documents should be submitted with the CHECK-LIST
2A (Appendix 7) for each application.
b) If Proforma Invoice / Purchase Order / Indent / Contract is mentioned in C.O., please
submit a copy of the said documents(s) as supporting document for Chamber’s
retention.
c) All photocopied documents should be duly certified by the authorised
signatory of the applicant’s company.

d) Invoices, Packing Lists and documents whatsoever required for Chamber’s
endorsement must be supported by written request of the related authority /
department.

2. Certification on Specific Requirement(s) of L.C.


You will ONLY NEED to provide us with the relevant documents should your Letter of
Credit (L.C.) require specific certification on
(i) Insurance Agent Declaration Form
Require Letter of Indemnity from Insurance Company (Duly stamped at any
Government Stamping Office)
(ii) Appended Declaration to Bill of Lading / Shipping Certificate
Require Letter of Indemnity from Shipping Company (Duly stamped at any
Government Stamping Office)
(iii) Percentage of Product’s Contents / Origin
If applicant states the following clauses in the C.O. or Invoice as part of the
requirements of L.C., applicant should furnish us either (i) An Auditor’s Report
certifying the percentage of product’s contents which is of Malaysian origin (Costing
Composition need not be attached); OR (ii) A Statutory Declaration
e.g.1 : “more than _ percentage of the raw materials and manpower used are
from country of origin”

e.g.2 : “the goods exported are solely / wholly / purely / 100% of domestic origin”

(iv) Production Date and / or Expiry Date


If applicant states Production Date and / or Expiry Date in the C.O. or other
documents, applicant MUST provide either
1. A Health Certificate from the Ministry of Health confirming to that effect; OR
2. A Statutory Declaration

(v) Others
Should the applicant’s L.C. require any other form of specific certification which could
not be substantiated by documents, the applicant MUST furnish us a Statutory
Declaration to that effect

e.g.1 : “…….there is no raw materials of Israeli origin in the manufacturing of ….”

3. Notes
(i) KLSCCCI would not consider C.O. application for shipments exceeding one
(1) month from the date of export.
(ii) KLSCCCI would not consider any advance C.O. application (before the date of
export) except for shipments by Air.
(iii)KLSCCCI reserves the rights to endorse or reject any application of C.O. at its
sole discretion.
