ABOUT TIMOTHY:  Timothy is a 60-year-old man. He is with his pet cat.


 Timothy aunty, Wendy, and daughter, Nicole, provide
informal support to him such as taking him for outing /
medical appointments.
 Tim has 2 children: Steve and Nicole. Tim has a closed
relationship with his daughter whom he calls on a daily
basis.
 enjoys listening to music.
 Timothy also enjoys talking on the phone with his daughter
every day.

provide to the medical team if medical advice is sought.

Timothy is wearing a Libre 2 sensor. The sugar reading is Sensor Glucose (SG) not blood glucose. It is important
when SG’s are unstable (hypo/hyper or sick days) that blood glucose should be performed to ensure SG’s are
correct.

Outpatient appointments for diabetes review will be arranged at diabetes centre.

Appointment with GP at least 3 monthly for diabetes review (bloods, urine, care planning). Tim is eligible for a
Chronic Conditions Management Plan and referral to Allied health Services such as podiatry billed to Medicare
(there may be a gap fee for these providers).

Timo should see an optometrist annually unless there are any eye problems, and the optometrist will advise if more
frequent reviews are required or refer to an ophthalmologist.

2. Wait 10-15 minutes then

are above 4 mmol/L,
continue to step 3.
3. If the next meal is more than
20 minutes away, give the
person 15 g of longer acting
carbohydrate. This is
equivalent to one of the
following:
 A slice of wholemeal or
multigrain bread
 1 small tub of yoghurt
 1 piece o fruit
 1 glass of milk or soy
milk
4. Wait, and ensure the person
has recovered fully before
continuing with usual
activities.
