CONFIDENTIALITY AND NON-DISCLOSURE AGREEMENT

This Confidentiality and Non-Disclosure Agreement (the "Agreement") is entered into by and
between:

The SCIENCE EDUCATION INSTITUTE, a service agency of the Department of Science and
Technology, with principal office address at Levels 1 and 2, Science Heritage Building, DOST
Compound, General Santos Avenue, Bicutan, Taguig City, represented herein by its Director IV,
DR. JAYEEL S. CORNELIO;

and

NICHOLE ANGELICA J. VERENDIA


_______________________________, of legal age, Filipino, ongoing
Polytechnic University of the Philippines -
Mabini Campus
undergraduate DOST-SEI Scholar enrolled at ____________________________, and with residence address at
Caloocan City
____________________________________________ for the purpose of preventing the unauthorized
disclosure of confidential information of the 2025 DOST-SEI Undergraduate Scholarship
Examination.
WHEREAS, the Parties agree to enter into a confidential relationship with respect to the
disclosure of confidential information derived from the 2025 DOST-SEI Undergraduate Scholarship
Examination to be held on April 5-6, 2025.
WHEREAS NICHOLE ANGELICA J. VERENDIA
_________________________ will act as SCHOLAR-VOLUNTEER for the 2025
DOST-SEI Undergraduate Scholarship Examination under the following terms:

I. That he/she understands that everything he/she received and accessed from the
activity is the property of the Department of Science and Technology – Science
Education Institute and should be treated with utmost confidentiality;
II. That he/she commits not to copy, reproduce, photograph, share, tamper, and
disseminate any part of the materials, information, document, and output derived from
the above-mentioned activity;
III. That he/she obligates himself/herself to use the materials only within the duration of
the activity until its completion; and
IV. That he/she understands that if he/she has been found to have violated the conditions
set for the service rendered, he/she will be held accountable for his/her actions.

NOW THEREFORE, for and in consideration of the foregoing premises, the PARTIES hereby
agree and bind themselves to the terms of the Confidentiality and Non-Disclosure Agreement.
