1.Total Direct Issue Quantity and payment made there of Direct Issue
Quantity under all Divisional Offices of FCI RO UP Region as below
mentioned from 2015 to now till date: -
A. Depot-wise (Name of the Depot & Distance from Railhead &Direct Issue
Quantity Rake-wise),
B. Month-wise (Total Quantity Directly Issued for each Month of above-
mentioned Depot as mentioned above in A).
C. Direct Issue Quantity payment made of above-mentioned (A&B).
D. Distance of nearest Depot to Railhead for all mentioned Depot of UP
Region and also mention the Depot name and distance from Railhead to
Depot/WB were Direct Issue Operations had been performed.
E. Also provide for each Depot of UP Region Transportation cost from
Railhead to Depot (FSD/SWC/CWC/PEG) separately and handling cost of
Stacking separately at each of the FSD/SWC/CWC/PEG) from 2015 to till
date.
F. Fifo violation observed in the Depot by any Authority in Direct Issue
Operations at each of the Depot Operations of UP Region from 2015 to till
 date. (Report Copy)
G. Designation of the Sanctioning Authority who authorizes and sanction the
Direct Issue Payment/HTC payment.
