CASE #1

Mrs. Gregg is a 55-year-old widow who has had an active career as a lawyer. She is an avid
reader. She is an independent woman who has had to adapt to many changes in her life. She
enjoys crafts and sings in the choir at her church. She has an adult son and daughter, and two
grandchildren who visit her frequently. She lives in her own home, a small bungalow in the
city.

You are assigned to give care and support to Mrs. Gregg, who had a stroke two weeks ago.
She is paralyzed on the left side and has difficulty speaking. She is able to hear well and can
understand when you speak to her. The long-term goals for her care include:

• regaining mobility
• regaining speech
• being as independent as possible
• preventing complications such as contractures and pressure sores

You are required to assist her to participate fully in all activities of daily living, and assist to
meet her physical, safety, and psychosocial needs.

1. What information would you want to find out about Mrs. Gregg and her home before
beginning her care? (2 marks)
As a support worker, I would want to know the location of where she lived (close to stores, and hospitals, appointments etc.).
I would__________________________________________________________________
want to know the layout of the house and if there were any safety concerns that should be addressed to avoid
anything happening to Mrs. Gregg when she is by herself. I would also want to know if she had any devices to help with her
__________________________________________________________________
speech or mobility such as an I PAD or a notebook to write things down on for the support worker or a walker/wheelchair to
__________________________________________________________________
get around the house. It would also be important to note any physical changes happening to Mrs. Gregg, such as, any
__________________________________________________________________
pressure sores currently developed.

2. List the types of information that you would expect to obtain from the care plan. (2 marks)
From the care plan, I would hope to find her diagnose(s), her treatment plan
(regaining mobility and speech, independence and preventing any damage to the skin
__________________________________________________________________
should be listed in this section), any orders that would be from doctors or nurses and
__________________________________________________________________
the plan of what type of care is going to be provided, for how long, and by who. It
__________________________________________________________________
should also contain the outcomes that are to be achieved by the support worker and
the client.
__________________________________________________________________

3. In the following problem, explain briefly the feeling expressed by the client and the actions
for the Personal Support Worker to take (2 marks each).

Situation 1

Mrs. Gregg continually uses her knife to eat with and is generally unable to choose the
correct utensil and unable to cut her meat. She has difficulty getting food to her
mouth. She resists help. One day she threw her plate across the room.

Client’s Feelings

in her mouth and she may be confused when support workers try and help her
_________________________________________________________________
as she thinks she has the correct utensil in her hand to make eating the meal

easier and more efficient

PSW Actions (physical and emotional help)

The PSW physical actions would be to not allow a knife to be at the table setting. The PSW should take away
any sharp _________________________________________________________________
utensils or cutlery out of the reach from Mrs. Gregg so as not to injury herself. The PSW would also
help clean_________________________________________________________________
up the mess from the plate that was thrown across the room so that Mrs. Gregg does not
endanger herself when she is by herself. Emotional help by the PSW would be to listen to why Mrs. Gregg is
_________________________________________________________________
angry, understand and make suggestions on how Mrs. Gregg could still be independent but in a safer manner.
_________________________________________________________________
Her safety is the most important but Mrs. Greg should also feel like she is being understood, not judged and
encouraged rather than not allowing her to complete these tasks on her own.

Situation 2

Mrs. Gregg has expressed a desire to read, but she is unable to understand the text in
her old books. She asks you to help her practice words. She says that she has trouble
passing the time and that she expects to return to work in several weeks.

Client’s Feelings
Mrs. Gregg most likely is feeling hopeless and upset that she has a desire to complete something
she is interested in but is unable to complete that by herself. Mrs. Gregg might also feel
___________________________________________________________________
embarrassment because she has to ask her support worker for some assistance with something
___________________________________________________________________
that she used to be able to do without any help.

Mrs. Green practice the words she would not be able to understand or read on her own. The
support worker should support the client when the client needs the support. Emotional help would include listening to
___________________________________________________________________
Mrs. Gregg but also___________________________________________________________________
trying to sympathize as to why she would feel the way she does. Emotional help can also include
the encouragement___________________________________________________________________
to Mrs. Gregg that she is able to do this, the reassurance that you as the support worker is there if
the help is needed and of course the compassion that you truly care to help, is also very important. If the policy of the
company and the client allow it, physical touch like a pat on the back, or a touch on the hand can also be encouraging
to the client

Situation 3

One day when you approach Mrs. Gregg, she is crying and says she doesn’t want to
get up. She says, “None of this should have happened to me. What did I do to deserve
this?”

Client’s Feelings
The client is sad, upset, angry, and frustrated that she has to go through what she is going through. Mrs.
_________________________________________________________________
Gregg would have gone through a lot with her husband passing away and having adult children who
might require help _________________________________________________________________
from time to time from their mom. They might also need help with the grandchildren,so
Mrs. Gregg would feel like she not does not time to herself to do the things she enjoys to do and be able to
_________________________________________________________________
do those things without support from anyone else. Mrs. Greg's would feel emotional due to not
_________________________________________________________________
understanding why all the bad things have happened to her and how she doesn't deserve to go through
something like this
PSW Actions (physical and emotional help)
The PSW would physically help Mrs. Gregg with whatever she wanted but also taking a step back to understand
that Mrs. Gregg is upset that she wishes she could do this on her own. The PSW could
_________________________________________________________________
physically help clean her up, wash her face, give her a hug, expressing communication by touch and showing that
you care and_________________________________________________________________
are there. The emotional help is listening, taking that step back and understanding how this would
make Mrs. Gregg feel. Mrs. Gregg would have a lot of different emotions and may feel like this from time to time, so
_________________________________________________________________
as the PSW or support worker, it is our job to listen to the client and showing that you care but to also help them
wherever they_________________________________________________________________
might not be able to do it on their own. It would be important to remember that Mrs. Gregg is still a
person with feelings

Situation 4

Mrs. Gregg insists on getting up by herself and moving independently from her chair to
the bathroom. She uses a walker. Occasionally she falls. She is a private person and
does not want you to attend her in the bathroom.

Client’s Feelings
The client may feel like she is not able to be independent. She insists on getting up on her own because she doesn't want the help as she
wants her independence_________________________________________________________________
as much as possible. She also might feel embarrassed that she needs to ask for the help but also may get
physically nervous to have to go in front of someone, that she may not be able to use the bathroom. She may want to have her privacy
_________________________________________________________________
but it also may because _________________________________________________________________
she wants to keep as much as her independence as she can
_________________________________________________________________

PSW Actions (physical and emotional help)

The PSW would support Mrs. Gregg and encourage her to use her walker. The PSW could say that it is for the
safety_________________________________________________________________
for herself and it could be dangerous if she has a fall. The PSW could offer to stand outside the
bathroom door or leave the door open a crack while standing in a room near the bathroom. The PSW should
_________________________________________________________________
encourage that if Mrs. Gregg wants to be in the bathroom by herself, the SW should stand outside the door in
_________________________________________________________________
case anything happens and help can be immediate. Emotionally, the SW would listen and try to understand
why Mrs. Gregg wants to be more independent but explain to Mrs. Gregg in a soft tone that it's for the safety

of everyone involved

4. Using all of the above information, describe how the following factors might affect Mrs.
Gregg’s relearning of speech and mobility. Use case study information to help answer
questions. (3 marks)

a. Age
Mrs. Gregg's age might affect this as it is harder to re-learn mobility and speech
_________________________________________________________________
the older a client is. It is more difficult to retain new information the older
_________________________________________________________________
someone is. It is also harder to stabilize balance and mobility has the body
_________________________________________________________________
generally decreases as the person begins to age
_________________________________________________________________

b. Individual characteristics: personality

_________________________________________________________________
Mrs. Gregg's personality might affect her with the re-learning of her speech and her
mobility_________________________________________________________________
because she wants to be independent, she doesn't understand why this
happened to her and she wants to complete most of her care on her own. She may be
_________________________________________________________________
limited on what she is able to do but with her personality, she might not accept that right
away and try to retain her dignity by doing all her care by herself
_________________________________________________________________

c. Environment
_________________________________________________________________
Mrs. Gregg's environment might affect her relearning her speech and regaining her mobility
_________________________________________________________________
because_________________________________________________________________
she may not have the space in the house to complete this task to 100%. She may
need help getting up or down the stairs, walking down the hallways with her walker and
_________________________________________________________________
getting in and out of different rooms with the way the house is designed and developed. She
may not have the equipment in her environment to help her with her speech and she may not
have enough support when the PSW is not there to help her with these tasks
5. Describe five (5) ways in which the PSW can promote Mrs. Gregg’s self-esteem and
motivation during her daily routines. (5 marks)
a. Providing companionship and comfort when the client needs it.
b. Promoting the clients independence (allow Mrs. Gregg to complete the tasks that
a. she is still capable of doing on her own so she doesn't feel like she is losing her
______________________________________________________________
independence).
b. c.______________________________________________________________
Think positively and encourage your client to do the same as much as possible
but respecting them and how they feel at the same time.
c. d.______________________________________________________________
Providing emotional support where needed and showing that you care and are
listening to the feelings of the client.
d. ______________________________________________________________
e.Being socially engaged; get the opinions and thoughts of your client on how to
e. complete
______________________________________________________________
care. Ask what they might like to do and the routine they normally
complete the steps in.

6. Define Restorative Care and what role it would play in helping with Mrs. Greg’s
rehabilitation. How could the PSW assist with this process? (2 marks)
Restorative care is the where the PSW is focused on helping the clients regain their independence to their fullest extent possible and to be
able to improve the quality of the client's life. Restorative care plays a huge role in helping Mrs. Greg's rehabilitation. It helps with the level
__________________________________________________________________
of Mrs. Gregg's functioning and what she is able to do on her own and what she needs help with. It can help Mrs. Gregg slow down and
__________________________________________________________________
complete her tasks with more ease. The PSW could assist with this process by helping Mrs. Gregg to carry out her daily living activities,
motivating and supporting her by showing her encouragement, respecting her and the goals she may have, as well as allowing her as
__________________________________________________________________
much independence as possible and following the care plan.
